Description
The Uniformed Services University of the Health Sciences (USUHS), Bethesda, MD requires the full preventive maintenance inspections (PMI’s), calibration, and unlimited repair services (including emergencies) for an ICP Quadrupole Mass Spectrometer, Excell model, S/N: 206 from the manufacturer Thermo Electron Corporation to be serviced at the Armed Forces Radiobiology Research Institute (AFRRI), Bethesda, MD. In addition to the full PMI’s the Government is requesting a one time initial repair. RESPONSE TIME FOR CORRECTIVE MAINTENACE: The contractor shall provide on-site repair support to correct all malfunctions, which may occur. Emergency service shall be available within 72 hours following contractor notification of a malfunction. The contractor will respond by telephone to emergency calls within two hours of notification by the Government. On-site response to emergency calls will be the next duty day and report to the Government. Remedial maintenance will be available during normal working hours, 7:00 to 4:00 weekdays, excluding government holidays. The contractor will respond on-site to remedial maintenance calls within 72 hours of notification by the Government. SPECIFIC TASKS: Contractor will provide unlimited consumable repair parts, field service labor, and travel for all repair requests. Online and telephone technical support will have a response time of less than two (2) hours. Online support services will be available to include software updates. The contract will also include the initial replacement of the RF generator and all associated labor and travel expenses to return the ICP Quadrupole Mass Spectrometer to full operation condition. The preventive maintenance inspection shall meet or exceed manufacturer’s latest established specifications and shall include, but not limited to, the following: (a) perform operational inspection on all systems and correct any deficiencies; (b) verify that the system’s internal and external calibrations are within manufacturer’s specifications; (c) software upgrades and or calibrations and repairs of software required for proper operational and system use; (d) replace worn tubing; (e) clean and lubricate mechanical parts in accordance with applicable manufacturer’s service manual; (f) perform an electrical inspection of the equipment and all accessories included in this contract, and make electrical adjustments as required to ensure compliance with manufacturer’s specifications; (g) replace excessively worn or defective parts. Excessively worn parts are those parts which may fail prior to the next preventive maintenance inspection visit if they are not replaced immediately; (h) visually inspect exterior of all systems for damage and cleanliness, and report any discrepancies to Logistics; (j) make mechanical adjustments to ensure proper operation; and (k) consult with the equipment operator regarding equipment performance. TELEPHONE ASSISTANCE: The contractor shall provide telephone assistance from 7:00 a.m. to 4:00 p.m., Monday through Friday (excluding Government holidays) for equipment covered under this contract. Telephone assistance shall be toll-free to the Government, and shall be manned by a qualified repair technician. REPLACEMENT PARTS: The Contractor shall use only new, original or factory refurbished replacement parts in the performance of this contract. If required replacement parts are not immediately available to the contractor while performing Preventive Maintenance Inspections or service calls, the contractor shall notify the Government of the deficiency, deliver and install all replacement parts required to complete the inspection to minimize downtime. The contractor shall not remove any government-owned equipment from AFRRI for service without the written consent from the Government. Should it be necessary to remove the equipment from the facility for servicing, the contractor shall provide, at no additional cost to the Government, an approved loaner item. In the event part(s) removed have no trade-in value the Government may elect to retain it for repair and future use, if a component, part, or system that has an AFRRI identifying number is replaced, the contractor shall notify the Government of any serial number and/or model number changes that result. DOWNTIME: Downtime shall commence at the time of the actual contact with the Contractor’s representative at the designated point of contact or with the Contractor’s answering service or other continuous telephone coverage provided for the Government’s use. Downtime shall end when the system and/or machine is returned to the Government in operable condition. In the event the system being maintained under the terms and conditions of this contract is moved to another location within AFRRI area, the terms and conditions of this contract shall continue to apply. If the system must be moved outside of AFRRI, then the continued applicability of this contract shall be subject to mutual agreement. SPARE PARTS/TEST EQUIPMENT REQUIREMENTS: Prior to performing any maintenance under this contract, the contractor must provide proof that he has the manufacturer’s recommended spare parts kit and the recommended test equipment required for preventive maintenance inspections and corrective maintenance. Also, show proof that he has access to and is able to obtain all repair parts and technical manuals/data currently available from the original equipment manufacturer. DOCUMENTATION: At the completion of each preventive maintenance inspections, remedial or emergency service calls, the contractor shall provide the Government a service report. Failure to provide a service report will result in payment delays until the service report is received. Service reports shall include: (1) type of service: preventive maintenance inspections, remedial or emergency calls; (2) description of services/repairs preformed to include the result of electrical safety test/inspection as per the National Fire Protection Agency 99 (NFPA 99) and description of parts replaced, quantity, part number and price(s) for parts; (3) signature of the service person who actually performed the repairs, preventive maintenance, labor rate per hour and number of hours spent during the service calls; (4) list of discrepancies which may still exist for any item under contract; (5) date and time that service was requested, started, and completed; (6) model, location, and serial number of equipment serviced; (7) medical equipment verification/certification: service technician shall provide certification documentation and shall provide a verification/certification label to be affixed to the medical equipment serviced. The label shall have the date the verification was completed and the next verification due date posted. All calibration, verification and certification services performed on medical equipment shall be subject to all Federal Acquisition Regulations (FAR), Federal, State, and Local safety requirements, manufacturer’s recommendations and must fully meet industry standards. This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is a request for proposal (RFP). The incorporated provisions and clauses are those in effect through FAC 2001-19. The NAICS code is 811219. This notice is being used as request for proposal for: (1) The contractor shall provide all personnel, equipment, tools, materials, transportation, parts, software, supervision, and other items and services necessary to perform an initial inspection, preventive maintenance inspections (PMI’s), calibration and unlimited repair services (including emergencies) as requested on an ICP Quadrupole Mass Spectrometer, Excell model, S/N: 206 in accordance with the terms and conditions as outlined in the Statement of Work. The period of performance will be for one year from the date the contract is signed. Performance will take place at Armed Forces Radiobiology Research Institute (AFRRI), 8901 Wisconsin Avenue, Bethesda, MD 20889. The following provisions apply to this acquisition and are required with your proposal: FAR Clause, 52-212-1, Instruction to Offerors - Commercial Items (OCT 2000), FAR Clause, 52.212-3 – Offeror Representations and Certifications –Commercial Items (JULY 2002), and the evaluation factors outlined in FAR Clause, 52.212-2, Evaluation – Commercial Items (JAN 1999), FAR Clause, 52.222-48, Exemption from Application of Service Contract Act Provisions for Contracts for Maintenance, Calibration, and/or Repair of Certain Information Technology, Scientific and Medical and/or Office and Business Equipment – Contractor Certification (AUG 1996) and the acknowledgment of amendments (if any). Telegraphic or facsimile offers are not acceptable. Period for acceptance of offers is amended to include the following: The offeror agrees to hold the prices with its offer firm for 90 calendar days from the date specified for receipt of offers. The provision of FAR 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The following evaluation criteria are included in paragraph (a) of the provision: Any resulting order will be issued to the company offering the best value to the Government, price and other factors considered. Factors that will be considered are Technical capability and price. Technical capabilities are significantly more important than price. Failure to provide the information relative to each evaluation factor may render offer non-responsive. 1) TECHNICAL CAPABILITY: Technical capability will be based on: (i) Qualifications: The Government requires that all repair technicians be fully qualified and show proof of certification. The offerors are required to show the strength and weaknesses of each employee/repair technician proposed for the performance of this contract; (ii) Preventive Maintenance (PM) Plan: Each offeror is required to provide a PM plan indicating the amount of maintenance to be performed, the steps required in performing this maintenance, diagnostic testing, replacement of parts, etc. The PM plan must provide the steps used to comply with OEM specifications or the Government's requirements and upgrades as stated in the statement of work. The proposed PM plan must show how it will be effective in providing all of the required maintenance so that equipment operational functions are not degraded. The plan must also show how maintenance will be performed with minimal disruption to normal operation of the laboratory equipment. The PM plan will be evaluated to determine if the Government can place a high degree of confidence in the soundness of the plan; (iii) Management Plan. The management plan sets forth the offeror's approach for efficiently managing the work as demonstrated by the proposed organization. The recognition of essential management functions, and the effective overall integration of these functions. The management plan describes the projected organization proposed for the work, including internal operations and lines of authority, together with external interfaces and relationships with the Government, major subcontractors and associate contractors; (iv) Past Performance: The Government will review the offeror's record of past performance and experience, or the lack thereof, considering the extent to which the offeror's record demonstrates ability to perform well the requirements of the contract, including it's ability to obtain and install OEM engineering and field change orders in a timely manner. Towards this end the Government will evaluate the depth, quality, breadth, relevancy and currency of the offeror's work experience. This will be considered in determining this sub-factor for conforming to the specifications and to standards of good workmanship, for adherence to contract schedules (including the administrative aspects of performance), for reasonable and cooperative behavior and commitment to customer satisfaction and for having a business-like concern for the interests of the customer. Technical acceptability will be determined solely on the content and merit of the information submitted in response to this provision as it compares to the minimum characteristics provided above. Therefore, it is essential that offerors provide sufficient technical literature, documentation, etc., in order for the Government evaluation team to make an adequate technical assessment of the proposal as meeting technical acceptability.
