Description
The Federal Bureau of Prisons (BOP) is soliciting quotations, restricted to small business concerns to format, print, collect and scan the BOP?s Prison Social Climate Survey (PSCS). The PSCS is used to collect Personal Safety and Security, Work Environment, Quality of Life, Personal Well-Being, Community Environment and Housing Preferences data from its on-site field staff. The contractor will: 1) format and produce surveys, that are image scannable, print and assign each survey a unique identification number; 2) supply return envelopes 3) prepare, print and distribute general instructions, along with the assembled, prepackaged bulk mail surveys, packing list and return envelopes to the designated contact person at each institution; 4) generate a report for each institution, which identifies by survey identification number which surveys they received, and a report; 5) receive and check-in surveys, scan and edit completed survey, and provide the test data, and 6) store the returned surveys for six months. The period of performance includes a base period of one year and four one-year option periods. Request for quotation, #0106-2004 is expected to be issued on or about March 5, 2004 and close 20 days thereafter, on March 25, 2004. Interested parties are expected to download the solicitation and subsequent amendments from this website. Hard copies will not be distributed. It is the responsibility of potential offerors to frequently check the site for amendments. All responsible small business sources may submit a request for quotation, which will be considered by the BOP. Faith Based and Community Organizations have the right to submit offers/bids equally with other organizations for contracts for which they are eligible. Contract award will be in accordance with Far Part 13, Simplified Acquisition Procedures, NAIC code 518210.
