Description

The Network Logistics Contracting Activity plans to negotiate on a sole source basis with Carolina Pain Specialist LLC, 2318 Sunset Blvd, West Columbia, SC, to provide a Nurse Practitioner services to eligible Veterans beneficiaries of the Augusta VA Medical Center, on as needed part time (Not to exceed 20 hours/week) basis, in accordance with the all terms, conditions and provisions of the solicitation. Services will be provided at the WJBD VA Medical Center. It is the VA's intention to solicit for a firm fixed price contract on a sole source basis. The contract includes a Base year (one year from date of award) plus 3 one-year renewal option periods, that may be exercised at the option the Government. The total duration of the contract(s) will not exceed 4 years. This solicitation is issued as a commercial item in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 13. RFQ 247-0262-04 is advertised with NAICS Code 621498 with the size standard of $8.5 Million. The solicitation will be issued o/a 3/19/04. The proposed contract action is for services which the Government intends to solicit and negotiate with only one source under the authority of FAR 6.302-1. Interested persons may identify their interest and capabilities to respond to the requirement or submit proposals. This notice of intent is not a request for competitive proposals. A determination by the Government not to compete to this proposed contract based upon responses to this notice is solely the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
