Description
This is a combined synopsis/solicitation notice for commercial items, prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This procurement is a 100% small business set-aside. Request For Quote on the following item: BLANKET: TWIN BLANKET, 13-14 OZ, SIMILAR TO SMARTFLEECE ULTRASOFT ITEM NO. 2509. SIZE: NO SMALLER THAN ?60 X ?80? OR LARGER THAN ?70? X ?90? DOUBLE NEEDLE/COVERSTITCH FINISHING MUST NOT WEAR RAPIDLY, SHRINK OR FADE MUST HAVE SUPERIOR STITCHING, ANTI-PILL POLAR FLEECE COLOR: DARK GREY QTY: 22,500 UNIT: EACH Contractor must quote on item listed. FOB: Destination for delivery is 2631 Gary Ave, Building 5789, Lackland AFB, TX 78236. Date and Time of Delivery: All 22,500 blankets must be delivered no later than 27 May 2004 between the hours of 7:00 a.m. and 4:00 p.m. CDST. This solicitation document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2001-20. The NAICS code for this acquisition is 313210 and the size standard is 1000 employees. Note that 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The following addenda have been added to clause 52.212-1(d): A product sample is required. 1 each blanket (size as stated above) must be shipped to: Dept of the Air Force, Attn: Pamla Wertz, 37 CONS/LGCBA, 1655 Selfridge, Lackland AFB TX 78236. Samples are due with submittal of offers and will not be returned to submitter. To determine susceptibility to shrinkage, color fading, napping, and loose stitching, samples will be subject to washing in hot water and drying on high heat at least 10 times; washing in cold water and drying on high heat at least 10 times; bed will be made up with blanket in the dust cover and linen exchange configuration before washing and following washing. Shrinking, fading, napping or loose stitching observed after washing will render the sample technically unacceptable. Far 52.212-2, Evaluation-Commercial Items, applies to this acquisition and the factors to be used are technical acceptability of product sample, price and required delivery. Offerors must include a completed copy of FAR 52.212-3, and Alternate I, Offeror Representations and Certification ? Commercial Items, with its offer. FAR 52.212-4, Contract Terms and Conditions ? Commercial Items, applies to this acquisition. The following addenda have been added to clause 52.212-4: ?Add the following to subparagraph (c): Additionally, issuance of modifications for an administrative change that does not affect the substantive rights of the parties (e.g., changes in paying office, appropriation data, funds increase/decrease, changes of address) shall not require the signature of the contractor and will be issued unilaterally.? The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items, applies to this solicitation. Specifically the following clauses cited are applicable: FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans; Far 52.222-36, Affirmative Action for Workers with Disabilities. All interested parties must be registered with the Central Contractor Registration (CCR) as prescribed in DFARS 252.204-7004 & 252.232-7009. CCR can be obtained by accessing the Internet (www.ccr.gov) or by calling 1-888-227-2423. The CCR number must be obtained before award can be made. Quotations on this solicitation must be received by 18 Mar 2004, 2:30 PM (CSDT).
