Description

Description: This is a combined synopsis/solicitation for a commercial service, prepared in accordance with the format in FAR Subpart 12.6, and supplemented with additional information included in this notice. This announcement constitutes the only s olicitation. Quotes are requested; however, a written solicitation will not be issued. Solicitation number W911S8-04-T-0022 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effec t through Federal Acquisition Circular 2001-03. The requirement is small business set-aside with a NAICS Code of 512290, SIC Code 7389, and size standard of 4 mil. The description of the commercial service is as follows: Walk through is required by March 1 6, 2004, please contact Chaplin Collins at 253-967-3686. Please contract Jacqueline Edgerton at jacqueline.edgerton@lewis.army.mil Statement of Work Four Chaplains Memorial Chapel Multi-media System. 1. LCD projector, mounted in the sanctuary, plugged into existing electrical outlets, wired to existing sound system, VCR/DVD installed with existing Multi-media system. System wired with switching capability to project, VCR/DVD, computer graphics from laptop computer, and auxiliary video through projector, and sound through existing sound/multi-media system. Additional capability to plug in laptop computer at front of sanctuary so an individual can run his own computer graphics slides. With appropriate long-throw lens. 2. Tensioned video screen, mounted in the front of the sanctuary. Screen must exactly match projected image at the distance from the projector = no white and no overlap of the image. 3. Installed in Four Chaplains Memorial Chapel, Fort Lewis. Current technical standard requires 2 projectors and 2 screens. Award will be made on an all or none basis. Provision 52.212-1, Instructions to Offertory-Commercial Service, applies to this acquisition. An addendum to this provision is applicable, in which the term offer or (whenever it appears) shall be repla ced with the term quoter. Quoter are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes. Also, please provide Federal Tax ID Number FAR clause 52.212-4, Cont ract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. Far clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.212-5 (b) are applicable to this acquisition: 52.222.21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33,52.222-41,52.222-42,52.222-43.
