Description

The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, TX intends to make a single award for the provision of Phlebotomy Technician services (North American Industrial Classification System (NAICS) code (621399 - $6 million) at the Federal Correctional Institution (FCI) Allenwood, Pennsylvania. The contractor shall be responsible for, but not limited to, maintenance of waiting list, and scheduling of patients; daily maintenance of laboratory equipment, including advising COTR when equipment requires professional maintenance or repair; biological specimen preparation and packaging for shipment to contract laboratories; maintenance of laboratory logs and other records as necessary for complying with CLIA guidelines; Contractor shall make medical record entries (on forms provided by the institution medical staff) that provide all information pertaining to the services rendered, i.e. specimen (s) obtained, and; reporting any problems arising in regards to laboratory services to the COTR or his designee. The Contractor shall provide one (1) Phlebotomy Technician who possesses a valid license as a Phlebotomist, and must provide a copy of his/her current license. The Phlebotomist must have completed either a 12 week Lab Technician program, an approved state program, or possess an associates degree. The contractor must have at least one (1) year experience as a phlebotomist or Lab Technician in a clinical setting. The Phlebotomist must possess good clerical skills, along with having the ability to read, understand, fluently speak, and legibly write English. Finally, the Contractor shall possess and maintain a current CPR certification. The license will be subject to verification. The Contractor will provide a copy of all diplomas and/or certificates to support stated qualifications. All contractors doing business with the Federal Government must be registered in the Central Contractor Registration (CCR) database. The website for registration is www.ccr.gov. The contract periods will consist of a 12-month base period and four 12-month option periods for renewal. The estimated quantities for the base and each option year are 1248 one (1) hour session per year. The solicitation will be available on/about April 2, 2004 and will be distributed solely through this website. Hard copies of the solicitation will not be available. The website provides downloading instructions to obtain all documents relating to this solicitation. All future information concerning this acquisition, including the solicitation and any subsequent amendments, will be distributed through the website. Interested parties are responsible for monitoring the website to ensure they have the most up-to-date information about the acquisition. The date for receipt of quotations is on/about April 30, 2004. This solicitation is a total small business set-aside. All responsible small business concerns are encouraged to submit a quotation which may be considered. See Note 1.
