- PAVING PROJECT, SAN XAVIER CAMPUS, TUCSON ARIZONA, RFQ-HHSI161200400012C. This project consist of repaving the existing drives and parking areas using a geomembrane fabric, paving a new dental parking lot, widening the roads to accommodate two way traffic, and landscaping around the dental building at the San Xavier Campus, Tucson, Arizona. The intent of the project is to: Extend the life of the existing paved areas, construct a new parking area for the dental patients and staff, widen the roadway to accommodate two way traffic, provide an anesthetically pleasing site for patients, visitors and staff. To accomplish this, the finished asphalt shall have a neat, clean and smooth appearance, allow maximum contractor interaction on the job site to increase coordination and expedite construction. BID BONDS, PAYMENT AND PERFORMANCE BONDS ARE REQUIRED. NAICS CODE IS: 234110 Highway and Street Construction. SIZE STANDARD IS $27.5 million for each. The estimated project size is between $100,000.00 and $500,000.00. SET-ASIDE NOTICE: This procurement is set aside under the Small Business Competitiveness Demonstration Program, FAR Part 19, as unrestricted open and full competition. MANDATORY REGISTRATION REQUIREMENT: FAR 52.204-7 Central Contractor Registration: Any and all firms or persons wishing to respond to this solicitation must be registered in the "Central Contractor Registration (CCR) database" which is the primary Government repository for Contractor information required for the conduct of business with the Government. Offerors and Contractors may obtain information on registration and annual confirmation requirements via the internet at http://www.ccr.gov or by calling 1-888-227-2423, or 269-961-5757. By submission of an offer, the offeror acknowledges the requirement that a prospective award shall be registered in the CCR database prior to award, during performance, and through final payment of any contract. Offeror must be currently licensed by the Arizona Registrar of Contractors in one of the following classifications A GENERAL ENGINEERING, ASPHALT PAVING A-14 A COPY OF THE OFFERORS CURRENT ACTIVE CONTRACTOR?S LICENSE MUST BE SUBMITTED BY THE DUE DATE OF THE PROPOSAL. Competitive formal Best Value Source Selection process in accordance with PART 15 of the Federal Acquisition Regulation (FAR) will be used to ensure selection of the source evidencing the best overall capability to perform the work in a manner most advantageous to the Government, as determined by evaluation of proposals in accordance with the established criteria. COMLETE REQUIREMENTS AND EVALUATION CRITERIA ARE CONTAINED IN THE SOLICITATION DOCUMENTS. A copy of the solicitation plans and drawings may be purchased for a non-refundable fee of $50.00 per set. A cashier check, bank or postal money order must be made payable to the Indian Health Service and must accompany your request. CASH, COMPANY CHECKS OR PERSONAL CHECKS WILL NOT BE ACCEPTED AND WILL BE RETURNED. Submit your written request for plans and specifications along with payment to Chief Contracting Officer, Indian Health Service, Division of Engineering Service, 1301 Young Street, Suite 1071, Dallas, Texas 75202-5433. A PRE-POPOSAL CONFERENCE IS SCHEDULED FOR April 14, 2004, at 10:00 am. All interested parties should meet at the San Xavier Health Center, Building SX-10, Tucson, Arizona. THIS IS THE ONLY AUTHORIZED SITE VISIT. Qualification packages and price proposals are to be sent to: Mr. William E. All qualification packages and price proposals must be submitted not later than 2:00 pm (Dallas time), April 29, 2004.
