Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation N68096-04-T-0001 is issued as an invitation to request for quotation. This solicitation is 100% set aside for Small Business. This solicitation is issued on a restricted basis as a total small business set aside under NAICS Number 421450. DESCRIPTION: Install Oxygen, Nitrious Oxide and Air Manifold and piping system at wing J-3 OR in Bldg.1, USNH, Guam. The new medical gas manifold will be located in room J320a and will run a piping system that provides medical gas outlets three each at the main operating room J-322, J326, J 327 and for room J316 PACU preparation and recovery room will have four medical gas outlets for oxygen only. The new medical gas system will consist of 12 tanks for Oxygen, 4 tanks for Nitrious Oxide, 4 tanks for air with automatic transfer of oxygen, Nitrious oxide and air reserved banks with flexible connectors, inclduing alarm panel system. Remove existing electrical disconnect switch and the capped off steam line, waterline metal cabinet in room J320a. Responsible to size piping, fitting and figure out the best layout system for the medical gas lines and outlets. Responsible for installation, testing and certification of the new medical gas system (High pressure cylinder oxygen, Nitrious Oxide and Air Manifold) including the alarm panel system. The alarm panel system shall be monitored at the Nursing station. All materials and workmanship shall confirm to the latest edition of NFPA 99 for Medical Health Care Facilities. CLIN: 0001. Testing of medical gas lines Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror-Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discoussions with offerors. Therefore, the offeror?s initial offer should contain the offeror?s best terms. The Government reserves the right to conduct a discussion if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision 52.212-3, Offeror Representation and Certification?Commercial Items. An authorized representative of the offeror must sign these represenations and certifications. The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions?Commercial Items. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items, applies to this acquisition.
