Description
This is a combined synopsis/solicitation. The GLAC (Great Lakes Acquisition Center) intends to negotiate a firm fixed price contract to provide PHYSICAL THERAPIST SERVICES for the Chicago Healthcare System ? West Side Division and on occasion the Lakeside Division. Request for Quotation #RFQ 69D-252-04 entitled ?Physical Therapy Services? is hereby issued for commercial items as full and open competition and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-20 and VAAR 97-10. NAICS Code 621340 applies for this procurement and the small business size standard is $6 million. This announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractor?s proposals (cost and technical) are herein requested. A response to requirements listed below shall be submitted on contractor?s letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Contractor?s price and technical proposal must contain the name; title and signature of person authorized to submit proposals/bids on behalf of the contractor. VA Chicago Health Care System has determined a requirement for two (2) Physical Therapists. The VA reserves the right to select therapists from different contractors resulting in multiple awards. The Physical Therapist is to work Monday through Friday 7:30 am to 4:00 pm or 8:00 am to 4:30 pm. The Therapist must be certified under The National Board for Certification in Physical Therapy, Inc. and hold a current license. VA will guarantee work starting April 1, 2004 through September 30, 2004. The following is a list of Duties and responsibilities: ASSESSMENT (1) Assessment for the presence of physical abnormalities. (2) Determination of function related problems including rationale or basis for making the assessment. (3) Development of individual physical therapy care plans. (4) Use of data to provide individualized treatment program. (5) Choice of therapy technique best suited to condition of patient to achieve maximum results. TREATMENT (1) Therapeutic exercises. (2) Joint mobilization. (3) Lower extremity prosthetic and orthotic review, fitting and training. (4) Pulmonary rehabilitation. (5) Cardiac rehabilitation. (6) Neuromuscular re-education. (7) Gait analysis and training. (8) Functional activity training. (9) Participation in pain care. DOCUMENTED TRAINING AND/OR CERTIFICATION (1) Assess the patient prior to the development of treatment plans and goals. (2) Provides plans in accordance with the initial evaluation findings for treatment aimed at prevention, reducing disability or pain, and restoring lost function. (3) Provides therapeutic interventions, which focus on posture, locomotion, strength, endurance, balance, coordination, joint mobility and flexibility. (4) Monitors the extent to which services have met the therapeutic goals relative to the initial and all subsequent examination. (5) Determines the degree to which improvement occurs and, when appropriate, revises the overall treatment plan. (6) Educates/counsels patients, families, and/or significant others regarding the disability incurred and about intervention reducing the existing disability, and improved management of any residual disability in the resumption of life roles in the home and community environment. (7) Provides clinical instruction for physical therapy students and physical therapy assistant students from American Physical Therapy Association approved schools. (8) Monitors activities of physical therapy assistants and volunteers. (9) Participates as an integral member of the interdisciplinary rehabilitation team. SPECIAL REQUIREMENTS: (1) Respects confidentiality and follows established policy regarding patient records. (2) Re-evaluates and upgrades treatment plan at intervals determine by Physical therapy department policy. (3) Documents in electronic medical record an initial, progress, and final evaluation that includes measurable goals, goals achieved and functional outcomes using the PT department documentation policy guidelines. Documents patient education according to VA policy. (4) Observes VA policies and procedures relating to infection control, Fire and Safety, Information Security, Patient Safety (mandatory training). (5) Maintains current CPR Certification. (6) Attends and participates in interdisciplinary team meetings. Communicates, consults and works cooperatively with other team members to provide comprehensive patient care. Consults with the Physical Therapy Chief, regarding specific clinical problems or questions. (7) Assists in the day-to-day operation of the department in routine housekeeping and equipment care that the department is responsible for. (8) Consults with patients and their families regarding educating, training and advising them about their disability/disease. Teaches/issues home programs to improve/maintain previous achieve goals. (9) Also pursues required continuing education as set by the registry. KNOWLEDGE REQUIRED BY THE POSITION: (1) Knowledge of patient care practices such as ? (i) Attitude and communication with veterans of various ages, dealing with their physical and mental limitation, safe lifting and transfer procedures, evaluating their physical needs, and infection control practices. (ii) Dealing with medical emergencies such as cardiac arrest, seizures, falls and violent behavior. (iii) Patient education for patients with altered states of consciousness, cultural diversity, communication of procedures, pre/during and post procedure. (iv) Has a thorough knowledge of and can apply principles of universal substance precautions. (2) Thorough knowledge of anatomy and physiology. A full-time equivalent (FTE) is defined as a contractor's employee working eight (8) hours per day, five (5) days per week (40 hours per week). Contractor?s employees must be present at VA and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. A VA record-keeping system of contractor hours worked is established through time and attendance logs in the departments being served. The payments to contract personnel for any leave, including sick leave, holiday, or vacation time, is the responsibility of the contractor. Leave granted to contract employees would be in accordance with contractor policies. VA is not obligated to the Contractor during therapist absence. Evaluation of proposals offered will include the following factors: Professional qualifications, clinical skills/interpersonal skills, availability, past performance and price. Award will be made to the offeror(s) determined to be the ?best value? to the Government. Awarded offeror(s) is hereby notified that the position would be considered ?low risk? under the positions sensitivity designate in accordance with IL 90-01-6 entitled Contractor Personnel Security Requirements. Evaluation may be made with or without discussions with the offeror(s).
