Description

Provide Community Based Outpatient Care Services in the area of Nephi, Utah from the Date of Award through August 31, 2005 with four one-year options to extend the term of the contract. The contractor shall provide for the delivery and management of preventive medical care for eligible enrolled veterans. The contractor is required to provide the facility, medical providers, equipment, supplies, and all administrative functions required for the level of care and range of services required. Services will be paid at a capitated rate and will be all inclusive. The contract will utilize simplified acquisition procedures in accordance with Commercial Items Part 12, and in accordance with Part 15, Contracting with negotiation. The issue date is O/A April 12, 2004 and the BOD is O/A May 14, 2004. The contract will be an IDIQ contract.
