Description
Minority Women?s Health Conference Focusing on Diabetes, Cardiovascular Disease, Cancer and/or HIV/AIDS Education PURPOSE: The Contractor shall provide professional services in developing, coordinating and implementing a one-day minority women?s health conference focusing on diabetes, cardiovascular disease, cancer and/or HIV/AIDS education. The primary intent is for the conference to target women from racial and ethnic minority populations (African American, Asian American, Pacific Islander, Hispanic/Latina American and Native American/Alaska Native). Eligible entities may include not for profit community based organizations, faith based organizations, national organizations, and tribal/urban Indian entities and organizations. BACKGROUND: The Department of Health and Human Services (DHHS), Office on Women?s Health (OWH) has as its mission the improvement of women?s health across the lifespan by directing, developing, stimulating, and coordinating women?s health research services, education and training. To that end, OWH has established public and private partnerships to address critical women?s health issues nationwide. STATEMENT OF WORK: 1. The Contractor shall develop, coordinate and implement a one-day minority women?s health conference targeting African American, Asian American, Pacific Islander, Hispanic/Latina American and Native American/Alaska Native and focusing on diabetes, cardiovascular disease cancer and/or HIV/AIDS education. 2. The Contractor shall develop an impact/outcome evaluation tool and assess the impact of the conference activities on eliminating health disparities as they relate to ?Healthy People 2010? and the DHHS ?Closing the Gap Campaign? (both these initiatives may be found on the DHHS web page located at: http://www.hhs.gov/. 3. The Contractor shall develop a process evaluation tool and assess the conference process and logistics. 4. The Contractor shall identify speakers and workshop facilitators who are experts in the conference disease areas, who are patient advocates and who have knowledge of the cultural norms and health beliefs of the targeted racial and ethnic groups. 5. The Contractor shall provide an initial progress report 2 months following the award of the contract, a mid-year progress report due 6 months after award of the contract and a final progress report due 12 months from award of the contract. The content of the reports is provided in the ?Deliverables and Performance? section below. 6. The Contractor shall promote the one-day conference in their prospective community(s) through local media and establish community stakeholder support from community leaders. DELIVERABLES AND PERFORMANCE: Period of Performance: The period of performance is for 12 months from award. Description of Deliverables: Following contract award, the Contractor shall provide the reports listed below at the times shown: 1. 2 months from award Initial Progress Report to include, but not limited to, the following: A comprehensive conference program summary describing the level(s) of prevention the conference will address (primary, secondary or tertiary), conference goals and objectives, address eliminating a health disparity(s), the significance of the minority group and it?s application to the conference and how the conference will relate to OWH and DHHS initiatives. 2. 6 Months from award Mid-Year Progress Report to include, but not limited to, the following: A program description, curricula, conference agenda report that describes the actual process/framework of the conference A logistics report describing all the logistical factors associated with implementing the conference (time, location, participation, recruitment, staff support, etc.) An evaluation tool to assess conference process and logistics (the effectiveness of implementing the conference program for the target audience) and an evaluation tool to assess impact/outcome (preexisting knowledge or skills versus post conference knowledge or skill) measures. A resource list describing all partnerships, speakers, materials, etc. used to plan the conference. An updated timeline/calendar describing when tasks for planning, implementing and evaluation will be complete. 3. 12 Months from award Final Progress Report to include, but not limited to a summary of the following: All logistical factors (time, location, participation and recruitment, staff support, etc. Resource information (partnerships, speakers, resource materials, etc. used to plan the conference. Activity materials (program, agendas, handouts, etc.) Timeline/Calendar highlighting the completion of all tasks related to the conference through the contract. Evaluation results based on both process and outcome measures. Implementation process (what happened, how it was done, when and why). Challenges and limitations. Action steps or a plan for action, results/implications of the conference and recommendations for further efforts. Project Officer The Project Officer is hereby designated to monitor the performance of this contract on behalf of the Government. The Project Officer will provide no supervisory or instructional assistance to Contractor personnel. The Project Officer?s function is primarily to provide the Contractor with working data. The PO is not empowered to make any commitments or authorized to make any changes that affect prices, terms or delivery as specified in this order. Any such proposed changes shall be brought to the immediate attention of the Contracting Officer for action. The acceptance of any changes by the Contractor without specific approval and written consent of the Contracting Officer shall be at the contractor?s own risk. PAYMENT AND INVOICING PROVISIONS: Payment Schedule The Government anticipated making three payments during the performance period. The Contractor may invoice following the delivery and acceptance of the three deliverables . Invoice Submission The Contractor shall submit invoices, in an original and two copies to the Project Officer. The PO shall submit the invoice and signed receiving report to the Program Support Center Financial Management Service, Division of Financial Operations, Parklawn Building, Room 16A-12, 5600 Fishers Lane, Rockville, MD 20857, Phone: (301) 443-3020 for payment. Invoices shall include the following minimum information: Contractor?s name and address Purchase order name Description of the services and price Invoice period Contractor?s Internal Revenue Service ?Tax Identification Number? (TIN) INSPECTION AND ACCEPTANCE: The PO as a duly authorized agent of the ordering/contracting officer is responsible for the review and acceptance of all deliverables.
