This announcement is published as a Total Small Business Set-Aside action per FAR 52.219-1 and 52.219-6 (this means you must be a small business, quoting the product of a small-business manufacturer); all eligible, responsible sources may submit an offer. This is a combined synopsis / solicitation for commercial items prepared in accordance with FAR 13 and the format in FAR subpart 12.6, as supplemented with additional information included in this notice. Solicitation number / RFQ N00244 -04-T-0475 applies; an electronic copy of the RFQ can be accessed at http://www.neco.navy.mil. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-20 and DFAR Change Notices effective through 3/23/04. The standard industrial code is 7338 (NAICS 561492) and the business size standard is $6 million. The agency need is for transcription service for the Trial Service Office West (TSO West). The requirement is for a fixed priced contract for one line item as follows. The statement of work is as follows: Transcripts will be prepared from audiotapes recorded on a Lanier or Sony recording system. Tapes will be monitored during hearings by Trial Service Office West (TSO West) court reporting personnel who will take notes as necessary to assist the transcriber in identifying the parties speaking on the ta pes. The number of tapes per record will vary. The Contractor shall certify that at least 35% of their transcriber's or the Contractor as an entity is qualified to serve as reporters or transcribers for U.S. Federal Courts, California State Courts, or San Diego county Municipal Courts. Contractor shall provide any specialized instructions and materials necessary to instruct TSO West staff personnel on type and form of case notes necessary to assist transcribers. Contractor shall provide tape pick-up and record delivery from the Quarterdeck of TSO West building 56 at Naval Station San Diego and must sign the Outsourcing Pick-up Log for each case received or delivered. Contractor shall be familiar with Naval terminology. Court-Martial transcripts will be prepared with strict adherence to the format prescribed by Appendix 14 of the Manual for Courts-Martial 2002 and the Na val Justice School Courts-Martial Record Preparation Guide. This mandatory record format requires a double-spaced page format that will result in records having a variable of lines per page of transcript. Transcripts for "non-courts-martial" proceedings, such as boards or courts of inquiry (BOI), may be prepared, at and for the Service's convenience, in a summarized single-space format. Transcription for courts-martial or other proceedings, are based on double-space paged or single-spaced for BOI. Contractor shall include cover sheets, title / caption, receipt, notice, and authentication pages with each completed record. Payments for service will be made with Government Purchase Card. Contractor shall ensure completed records contain no more than three (3) spelling, grammar, punctuation, form, substance, format, or typographical errors and omissions per page prior to return ing to final output to TSO West. Completed records containing more than three (3) errors or omissions on any single page shall be considered unacceptable and subject to re-performance by the Contractor at no additional charge to the Government. Contractor shall provide to TSO West one paper copy of the transcript and a 3 ?" disc in Microsoft Word format containing the transcript. Transcripts shall be completed within 10 - 14 days. The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items applies. Clause 52.212-4, Contract Terms and Conditions - Commercial Items applies as well as the following addendum clauses: FAR 219-1, Small Business Program Representations; FAR 52.219-6, Notice of Total Small Business Set-Aside; FAR 52.211-5, Material Requirements. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orde rs - Commercial Items applies with the following applicable clauses for paragraph (b): FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era, FAR 52.222-36, Affirmative Action for Workers with Disabilities; FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items, applies with the following clauses applicable for paragraph (b): DFARS 252.225-7001, Buy American Act and Balance of Payment Program; and DFARS 252.225-7012, Preference for Certain Domestic Commodities. OFFERORS ARE REQUIRED TO COMPLETE AND INCLUDE A COPY OF THE FOLLOWING PROVISIONS WITH THEIR PROPOSALS: FAR 52.212-3, Offeror Representation and Certifications - Comm ercial Items. The government intends to make a single award to the responsible offeror whose offer is the most advantageous to the government considering price and price-related factors. Provision 52.212-2, Evaluation - Commercial Items, applies with paragraph (a) completed as follows: Award will be made to the offeror that meets the solicitation's minimum criteria for technical acceptability at the lowest price. Parties responding to this solicitation may submit their quote in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc.) but must include the following informa tion: 1) Company's complete mailing and remittance addresses, discounts for prompt payment, if any (e.g. 1% 10 days), anticipated delivery / availability of product / s, the company's CAGE code, Dun & Bradstreet Number, and Taxpayer ID number. In addition, if you are quoting on a comparable commercial item, product literature must be included. All FAR certifications and representations specified above must also accompany your quote. Quotes must be received no later than 3:00 PM, local time on 4-2-04, and will only be accepted via e-mail @ (jessica.dunker@navy.mil) or at the following physical address: Fleet & Industrial Supply Center, 937 North Harbor Drive, 7th floor, San Diego CA 92132-0212.
