Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulations format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation (R6-15-04-80) is issued as a Request for Quotations (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-21. This acquisition is a 100% set-aside for small business. The small business size standard for this acquisition is $6.0 million and the NAICS code is 811213. It is anticipated that a Time and Materials, indefinite delivery/requirements, task order contract will be awarded from this RFQ with a minimum amount of $0, with an estimated amount of $15,000 and a maximum amount of $100,000 per one-year term. Period of performance will be for 12 months from the date of contract award with 2, one-year option periods to extend the term of the contract. The Umpqua National Forest seeks the services of a firm on a ?Call When Needed? basis for the maintenance, troubleshooting, and repair of existing telecommunications systems, equipment, wiring, and fiber optics on the Umpqua National Forest. The vendor shall furnish all supplies, materials (except when Government-furnished), labor, supervision and transportation required to complete task orders. Vendor shall provide certified/experienced technicians for the repair and maintenance of: Newbridge 3600 T1 Channel Bank; Mitel SX200D PBX; Nortel Meridian Option 11 PBX; Nortel Meridian Voice Mail. If not certified, technicians shall be certified within four months of contract award. The following types of telecommunications system failures can be expected and should be addressed in the offerors? proposals: Critical - complete failure of CPU, or any component which renders an entire telephone system inoperative; Emergency - when a failure involves more than eight (8) telephone instruments or one or more trunk circuits or failure of a common control unit, power supply, SMDR or accounting measuring system signal generating device, or attendant console which does not render the entire system inoperative; Routine - which is not a failure but maintenance such as station moves, changing numbers, and other modifications that do not meet the critical or emergency maintenance criteria. Sites where work is to be performed include the Supervisor?s Office 2900 N.W. Stewart Parkway, Roseburg, OR; Service Center, 2691 N.E. Diamond Lake Blvd., Roseburg, OR; North Umpqua Ranger Station, 18782 North Umpqua Highway, Glide, OR; Wolf Creek Job Corps Center, approximately 12 miles south of Glide up Little River Road; Toketee Ranger Station, approximately 45 miles east of Glide, OR on State Highway 138; Diamond Lake Visitor Information Center, approximately 67 miles east of Glide, OR on State Highway 138; Tiller Ranger Station, approximately 25 miles east of Canyonville, OR on County Highway 1 (Tiller-Trail Highway); Cottage Grove Ranger Station, 78405 Cedar Park Road, Cottage Grove, OR; Dorena Genetic Resource Center, 34963 Shoreview Road, Cottage Grove, OR; and, Powers Ranger Station, Highway 242, Powers, OR. The provision at 52.212-1, Instructions to Offerors?Commercial, applies to this acquisition. No special format is required for submission of proposals and may be submitted on letterhead stationery and at a minimum shall show: (1) Contractor?s Technical Capability including key personnel qualifications and experience, supplies, tools and equipment; (2) response time for each type of systems failure; (3) Contractor?s Past Performance History to include recent and relevant contracts for the same or similar services and other references (including contract numbers, points of contacts with telephone numbers and other relevant information); (4) Contractor?s pricing scheme for time, materials, travel, and discount terms in sufficient detail to evaluate compliance with the requirements as stated in this announcement including standard commercial published price lists and terms demonstrating most-favored customer status if available, for the Base Year and 2 option years for extending the term of the contract; (4) Representations and Certifications?Commercial Items at FAR 52.212-3. The proposal shall be signed by an official authorized to bind your organization. Offerors who fail to furnish required representations and certifications, or reject the terms and conditions of the solicitation, may be excluded from consideration. (ix)FAR 52.212-2, Evaluation--Commercial Items is amended to read: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be the best value for the Government, price and other factors considered. The following factors will be used to evaluate offers: (1) technical capability; (2) past performance; and, (3) price. Technical capability and past performance are generally more important than price, provided, the price proposal is deemed reasonable. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s); (c) a written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer?s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. All proposals shall be clearly marked with offeror?s name and RFQ number. The following Federal Acquisition Regulations (FAR) provisions and clauses apply to this solicitation and are incorporated by reference: FAR 52.212-1, Instructions to Offerors -- Commercial Items; FAR 52.212-3 Offeror Representations and Certifications--Commercial Items; FAR 52.212-4, Contract Terms and Conditions--Commercial Items; with addenda clauses FAR 52.216-18, Ordering - (a) date of award through any option to extend services, FAR 52.216-19, Order Limitations - (a) $0, (b)(1) $25,000, (b)(2) $100,000, or (b)(3) 7 days, (d) 10 days, FAR 52.216-21, Requirements - (f) 30 days after contract completion, FAR 52.245-5, Government Property (Cost-Reimbursement, Time-and-Material, or Labor-Hour Contracts), FAR 52.217-9, Option to Extend the Term of the Contract - (c) three years; and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders amended to read: ?no SCA wage decision applies since fewer that 5 Service Employees are to be employed (pursuant to 29 CFR 4.3a)?. ORDERING: Services and supplies to be provided herein will be ordered by issuance of a written task order. Each task order will contain the following information: (1) Task Order Number, (2) Date of Order, (3) Services and Supplies Ordered, and, (4) Work Locations. A list of authorized ordering officials will be provided upon award.
