Description

Contractor shall provide for the delivery and management of primary and preventive medical care and the continuity of that care to eligible beneficiaries of the Department of Veterans Affairs. The eligibility of veterans will be determined by the John D. Dingell VA Medical Center, Detroit, Michigan and confirmed in writing to the provider. The contractor shall be centrally located in the Pontiac, Michigan area to serve veterans anticipated to receive primary care services who reside int areas partially designated as Medically Underserved Areas/Medically Underserved Population and/or Health Professional Shortage.
