SOURCES SOUGHT NOTICE This notice is to request sources that have full and complete business capability in performing administration and management of a disclosure and examination operation that is located in the Washington, DC metro area. All activities are located at one site location. The tasks include receiving, examining and processing of financial type reports; receiving, processing, retrieving, securing and retiring of public disclosure and reporting files and other file reports to the public, government officials and government agencies;also, capability to prepare certified copies of public disclosure reports filed in strict accordance with Public Law or Act and related laws for the use of law enforcement officials in legal proceedings. Vendors should also possess the capability to perform high quality fast scanning, data imaging and data capture of reports both on-site or at an off-site location. The scanning is supplemented also be data key entry as necessary to ensure a high level of accuracy. The imaged reports and the reported data are made available for public disclosure on the Internet. Interested vendors should submit by EMAIL ONLY, the following: 1) Company Name 2) Company corporate official 3) Phone 3) Email 5) Complete Company address (must be in the DC metropolitan area only (includes VA and MD areas) MUST HAVE an actual physical location in case of site visit to confirm capability of operational status 6) Company Website 7) DUNNS NR 8) CAGE Code 9) TIN NR 10) 8(a) SBA certification number and date 11) SDB certified or not 12) and whether your firm is HubZone, WO, VOSB or not 13) Years Experience performing the described work 14) Years in Business 15) Total annual Revenue from this line of work 16) GSA Schedule Nr. (if any) 18) List ALL Federal and State government contracts, or PO your firm has performed under including contract numbers, PO numbers and dates of the contracts 19) List $$ amounts of contracts for each contract or PO 20) List whether you have any known Past Performance Data that can be verified in the Past Performance Information Retrieval (PPRIS) federal data system and if not whom your Past Perforamnce Reference POCs are and list them 21) indicate your firms willingness to BID or Quote on a Firm Fixed Price basis for specific workload.
