- THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. SOLICITATION NUMBER SSA-RFQ-04-0301 IS BEING ISSUED AS A REQUEST FOR QUOTATION (RFQ) USING SIMPLIFIED ACQUISITION PROCEDURES. This acquisition is not set aside for small businesses. The NAICS code is 423430 and the related small business standard size is 100 employees. The Social Security Administration (SSA) intends to renew the annual maintenance on the following Government-owned Adobe site licenses effective April 26, 2004 through April 25, 2005: a) SKU-651000254 for 79,000 each Adobe?s Form Client 5.0 WNT Premium; b) SKU-61050025 for 65,300 each of Document Server for Reader Extensions 5.0 WNT 20000 and up; and c) SKU-61050025 for 13,700 each of Document Server for Reader Extensions 5.0 UNX 20000 and up. The Adobe form filler software is used in field offices nationwide and in state Disability Determination System (DDS) offices for an electronic form solution to support the SSA?s Electronic Disability (eDIB) project. NOTE: The Document Server for Reader Extension is used simultaneously with the Form Client during a three (3) year transition period. The provisions at 52.212-1, Instructions to Offerors-Commercial Items, apply to this acquisition. The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The clause at 52.212-4 Contract Terms and Condition-Commercial Items applies to this acquisition. Additionally, the clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders?Commercial Items, including: (11) 52.222-21, (12) 52.222-26, (13) 52.222-35, (14) 52.222-36, (15) 52.222-37, (16) 52.225-3, applies to this acquisition. Offers must submit: (1) a written firm-fixed price quotation; (2) description of support with a toll free telephone number; and (3) a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items which may be downloaded from Http://www.arnet.gov/far/loadmin52.html.
