Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this noticeand with an intent to award on a sole source basis. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-04-T-0025 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 541990, Size: 6.0. This procurement is a small purchase, unrestricted solicitation. The contractor shall provide CLIN 0001: Non-personal Services for Rapid employment of N-Acetylcysteine Treatment for Otologic Recovery (REACTOR), a Prospective, Multicenter, Randomized, Double-blind, Parallel Group, Placebo-controlled Study Assessing the Efficiency of the Nutritional Supplement N-Acetylsteine Treatment of Acute Acoustic Trauma for the period of 1 October 2004 through December 1, 2004 to include: 1) Initial second quarter study site data collection, 2) Complete second quarter study data collection, 3) Provide real time data management and monthly database integrity monitoring, 4) Provide summary report of second quarter testing and data collection. Milestones: 1) Initial data collection of second quarter training events for all sites. 2) Completion of data collection for second quarter training events at all sites. 3) Required reports submitted to PI Dr. Ronald Jackson, 4) Provide summary report of second quarter testing and data collection. QTY: 01, Unit of Issue: LOT.
