Description

This is a request for proposals for the installation and site prep of a Philips Medical System - Allura 9CFD3 System. This is a turn-key project. Contractor must furnish all labor, materials, tools, equipment, expert supervision, A&E services, drawings, construction, materials, equipment and subcontractors as necessary to perform all work associated with the installation and operation of a Philips Medical System 9CFD3 System. This project includes all renovation work to the current areas which are now an exam room, control room, electrical equipment room and storage room. The VA Medical Center will provide a proposed plan detailed in a drawing provided by Philips Medical Systems. Evaluation factors will include Technical Capability, Past Performance, and Price. Contractor must be able to complete project within 75 days after notice to proceed. Solicitations will issue on Friday May 07, 2004 and Proposals will be due on Friday May 28, 2004 by 4:00 p.m. All request for proposals must be received in writing. Request for proposals can be requested via fax (504) 589-5289. Construction range is $100,000 - $250,000. The NAICS Code is 238990; $$12.0M is the size standard. This procurement is subject to the availability of funds.
