Description
The Fleet and Industrial Supply Center (FISC), Norfolk Acquisition Department intends to negotiate a firm fixed price contract utilizing the procedures identified at FAR Subpart 13.5, Test Program for Certain Commercial Items, on a sole source basis with VISX USA, Inc., 3400 Central Expressway, Santa Clara, CA, 95051 for 600 Vision Key Cards (VKC) and 1200 License Fees to be used with a Government owned VISX Excimer Laser Star S4. The statutory authority permitting the pursuit of this requirement in support of the Ophthalmology Department, Naval Medical Center, Portsmouth, VA on a sole source basis is 10 U.S.C. 2304(c)(1) as implemented by FAR 6.302-1. There is only one responsible source and no other supplies or services will satisfy agency requirements. A complete RFQ package will be available on or about 23 April 2004 by downloading the documents at the Neco Website as foll ows: http:// www.neco.navy.mil. The North American Industry Classification System (NAICS) code for this acquisition is 423460. Potential contractors are hereby notified that the solicitation and subsequent amendments to the solicitation will ONLY be available for download at the above site; this office will no longer issue hard copy solicitations. By submitting an offer, the vendor is self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred, or otherwise ineligible to receive contracts from any federal agency. Failure to do so may represent grounds for refusing to accept the offer. By submission of an offer, the contractor acknowledges the requirement that a prospective awardee must be registered in the CCR database prior to award, during performance, and through final payment of any contract resulting from this solicitation, except for awards to foreign vendors for work to be performed outside the United States. Lack of registration in the CCR database will make a contractor ineligible for award. Contractors may obtain information on registration and annual confirmation requirements by calling 1-888-227-2423, or via the Internet at http://www.ccr.gov. Numbered Note 22 applies.
