- 1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2) Scope of Work: Diabetes/Teleophthmology Consultant/Case Manager California Area Indian Health Service Contract Period ? May 01, 2004 ? April 30, 2005 1000 hours/12 months Overall Project Goal: Improve the health status of Native American people with diabetes and Native Americans that are at risk for developing diabetes - Promote effective use of RPMS Diabetes Register databases at California Area Indian Health clinics - Provide clinic visits (1) within contract period at 20 California Indian health clinics (clinics listed below) - Provide on-site and remote teleophthmalogy support to sites using the retinal cameras (14 sites) - Provide ongoing telephone support - Provide ongoing remote on-line support throughout the contract period - Network with other contractors (California Area Office, Modcom, Cimarron, Ray Shields, MD, etc.) and build on previous projects (for example, Diabetes Register building, DMS training, EpiInfo training) - Act as a resource for the annual IHS diabetes audit (assist with running the audit, transmitting the audit to CAO, and correcting errors within epiinfo) - Assist clinics in setting goals for improving the annual diabetes audit/care - Assist and provide support in setting up internal periodic diabetes care inquiries valuable to individual clinics (for example, list of clients with HbA1c values over 9.0 for targeted intervention) - Promote ADA and IHS Standards of Diabetes Care - Promote and support a multi-disciplinary team approach and case management for diabetes care - Recommend professional staff acquire Certified Diabetes Educator (CDE) credential - Promote IHS Recognition or ADA accreditation of diabetes education programs - Provide support in establishing goals for clinic use of databases - Advocate clinic commitment to diabetes care and use of RPMS databases by working with key clinic administrators, key staff, key Health Board members, and key community members - Conduct comparison of electronic data vs. medical record for accuracy of diabetes reporting - Act as a nutrition education resource; promote health eating and activity lifestyles for diabetes and diabetes prevention - Complete monthly reports the California Area/Area Diabetes Consultant (ADC) in addition to reports after individual clinic visits - Assist CAO ADC in other activities as needed Activities to achieve goals: - Clinic visit (1) within contract period at 20 California Indian Health clinics (clinics using RPMS and accepting assistance); at clinic visit assess: - Clinic organization - diabetes program structure (e.g. staff involved, education program, staff who enter diabetes-related data) - use of Diabetes Management System and DM Register RPMS and QMAN reports to assist in updating register, evaluate patient status categories, etc. - last IHS diabetes audit and run current audit on active clients - case management system, if any; devise system and assist in implementation as needed - review education needs (diabetes knowledge, RPMS, DMS, and EpiInfo) - assessment of eye screening process using teleophthmalogy Knowledge and Skills Required Ability to obtain CDE credential with-in 12 months Healthcare professional status (Registered Dietitian) Experience working with the RPMS/DM Management databases Experience working with teleophthmalogy Minimum 10 years experience in diabetes care Knowledge of Indian Health Service, Tribal and Urban Indian healthcare programs and health needs of Native Americans Excellent communications skills, both verbal and written 19 California Area Indian Health Clinics 1) Bakersfield Urban Indian Health Project 2) Chapa De Indian Health 3) Feather River Indian Health (2-visits) 4) Fresno Urban Indian Health Board 5) Greenville Rancheria 6) Indian Health Council, Inc. 7) Karuk Tribal Health 8) K?IMAW: Medical Center 9) Lake County Indian Health 10) Northern Valley Indian Health 11) Pit River Indian Health 12) Riverside San Bernadino County Indian Health 13) Round Valley Indian Health 14) Sonoma County Indian Health 15) Shingle Springs 16) Tule River Indian Health 17) Toiyabe Indian Health Project 18) Tuolumne Rural Indian Health Board 19) United Indian Health Services, Inc. 3) Offeror must submit name of individual to perform work and provide and provide documentation of all related qualifications. 4) Award will be made on best value based upon price and qualifications.
