Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation only; proposals are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation is issued as a Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2001-14, Defense Acquisition Circular (DAC) 91-13 and Air Force Acquisition Circular (AFAC) 2003-0501, and can be obtained at http://farsite.hill.af.mil/. The North American Industrial Classification Code is: 611***** QUOTES MUST BE GOOD TILL 23 May 2004 ***** The BID SCHEDULE shall be for Line Item 0001: The contractor will be required to provide a summer camp program for military family members, commencing July 12th 04-Aug 6th 04. The contractor will provide 4 counselors that will plan, design and implement a full day camp for a total of 4 weeks. Camp counselors are required to arrive the week of 5 July 2004 for site orientation by the Youth director, or assigned alternate. The summer camp program will provide activities to meet the social, creative and physical development needs of military members? children from 6-12years. An example of activities would include but not limited to: Arts and Crafts, Sports and fitness activities, e.g. bowling, Water activities, swimming in the installation pool/at the beach and music performing arts. The counselors will provide a camp within the hours 0900 -1700 Monday ? Friday for no more that 48 youth, aged 6-12 years. Camp counselors will maintain a 1:12 adult to child ratio and 1:6 adult ratio for water activities. Required certifications: A minimum of two years of college or two years experience preferred in elementary school guidance or education, social work, psychology, child and/or youth development, human development, sociology, or youth recreation. Camp provider must comply with HQ/AFSVA/SVPAC Airforce School Age Program Standards. Staff must be trained in CPR and First Aid. Staff must be trained in Red Cross basic Lifeguarding and or Emergency Water Safety. Staff must comply with the requirement of a State Criminal history Repository check and or a National Agency check. Contracted employees who have previously received a background check must provide proof of the check or obtain a new one. Staff must have a physical examination within the last 3 years and have the required immunizations for this overseas location. The following factors shall be used to evaluate offers: 1) Past performance in accordance with FAR 15.304 2) Technical capability of the contractor to meet the Government requirement. 3) Cost to the Government for the Youth Service. The order of importance for evaluation criteria in descending order of importance is: past performance, technical capability, and price. (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
