Description

The Centers for Disease Control and Prevention (CDC) intends to award a contract which provides a focused intervention training program and ongoing technical assistance devoted to building the capacity of public health workers, community organizations and institutions as well as community members to identify implement and evaluate public health interventions and best practices intended to prevent and reduce Diabetes and its associated risk factors and complications. The contract will also provide for a number of established core trainings that will assure the continuation of essential infrastructure necessary for continued participatory, community-based diabetes prevention and control programs. CDC anticipates the award of a cost reimbursement contract, with a 60-month period of performance. Questions regarding the stated requirement shall be addressed to the Contract Specialist via e-mail at tdc0@cdc.gov; offerors shall note Request for Proposal No. 2004-N-01221 in the subject line of the e-mail. A consolidated question and answer listing will be posted to this site. The solicitation will be posted to this site no or about 5/11/04 and proposals are due on or about 6/14/04. CDC will consider all responsible sources. Offerors are expected to download all documents concerning this requirement, as no hard copies will be mailed.
