- Description
- The Bureau of Prisons is seeking a Request for Quote from sources having the ability to provide IN-PATIENT assessment and counseling services within the CCC in Fort Worth, Texas, located at 1424 Hemphill Street, Fort Worth, Texas 76104, for drug abuse treatment services for male and female Federal offenders (inmates) held under the authority of various United States Statutes. Performance period under this contract will be for a one year base period to begin on October 1, 2004 with four (4) one year option periods, with service ending (if all option years are renewed) on September 30, 2009. The services required for this contract include: Base Period: October 1, 2004 to September 30, 2005. Estimated Number of Inmates 30. (30) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 30 inmates = 1,440 total units; Group Counseling Sessions (30 minute sessions per inmate) 48 Group Units x 30 inmates = 1,440 total units. Family Counseling Sessions (30 minute sessions) 2 Family Units x 10 inmates = 20 total units. Option Year One: October 1, 2005 to September 30, 2006. Estimated Number of Inmates 32. (32) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 32 inmates = 1,536 total units; Group Counseling Sessions (30 minute sessions per inmate) 48 Group Units x 32 inmates = 1,536 total units. Family Counseling Sessions (30 minute sessions) 2 Family Units x 10 inmates = 20 total units. Option Year Two: October 1, 2006 to September 30, 2007. Estimated Number of inmates 34. (34) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 34 inmates = 1,632 total units; Group Counseling Sessions (30 minute sessions per inmate) 48 Group Units x 34 inmates = 1,632 total units. Family Counseling Sessions (30 minute sessions) 2 Family Units x 10 inmates = 20 total units. Option Year Three: Estimated Number of Inmates 36. (36) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 36 inmates = 1,728 total units; Group Counseling Sessions (30 minute sessions per inmate) 48 Group Units x 36 inmates = 1,728 total units. Family Counseling Sessions (30 minute sessions) 2 Family Units x 10 inmates = 20 total units. Option Year Four: October 1, 2008 to September 30, 2009. Estimated Number of inmates 38. (38) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 38 inmates = 1,824 total units; Group Counseling Sessions (30 minute sessions per inmate) 48 Group Units x 38 inmates = 1,824 total units. Family Counseling Sessions (30 minute sessions) 2 Family Units x 10 inmates = 20 total units. The services described shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work, dated January 2004. The estimated closing date is o/a June 10, 2004. All responsible sources may submit a quote which will be considered by this Agency. The RFQ will be posted o/a May 10, 2004.
