- Potential sources sought to participate in the regional standardization of Patient Exam Gowns, Capes and Drapes. TRICARE Pacific Region 12, a military integrated delivery network (IDN), comprised of Army, Navy, Air Force, Marine Corps and Coast Guard medical treatment facilities in Hawaii, Guam, Korea, Japan and Okinawa are in the process of standardizing Patient Exam Gowns, Capes and Drapes. If your company holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center Philadelphia (DSCP) for Patient Exam Gowns, Capes and Drapes, your company is eligible to participate in the standardization process. This is in pursuant to the DAPA clause by which DAPA holders agree to the standardization process. P lease respond by e-mail: susan.hwang@amedd.army.mil and provide your (1) company name, (2) address, (3) point of contact, (4) telephone number, (5) fax number, (6) e-mail address, and (7) answers to the below listed questions no later than May 21, 2004. For questions, please call (808) 433-3686. 1. What is your DAPA number? 2. Do you have a distribution agreement with our Prime Vendor Cardinal? 3. Can you provide a complete listing of the prime vendor order numbers for your products? 4. Can you service our entire region to include Military Treatment Facilities in Hawaii, Guam, Japan, Okinawa, and Korea? 5. Can you provide education materials, training and in servicing to all of the MTFs if requested? 6. Can you provide a MSDS (if required) for this product group? 7. Is your product line latex free? 8. What is your history of backorders or recalls? 9. What is your return goods policy? 10. Under what brand name do you manufacture or distribute your product? 11. What industry standards are utilized in the manufacturing your products? 12. Do you have an 800-customer service number or product representative available for all of our MTFs? 13. Provide a full description of your products. 14. Do your products meet the standards of flammability testing and the NFPA standards for fire safety? 15. What testing was used to support the efficacy of product as an exam gown, cape and modesty drape? 16. Can you supply all three categories: Exam cape style, full patient exam gown (neck to knees coverage) and half sheet modesty drape? 17. Are all of your products available in all-tissue or tissue/poly/tissue material? 18. What weave/bond are your products? 19. Are the gowns and capes available in X-large sizes?
