Description
This is a combined synopsis/solicitation, Request for Quotation 19-04-025 , for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented without additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 19. This is 100 percent small business set-aside. The NAIC code is 337124, the size standard for this code is 500 employees. The funding available to buy the following table is $78,000. We would like you to provide a quote to us on the quantity of tables you could provide for $78,000. Furnishing of 8-foot pre-manufactured picnic tables with seats, which shall meet the following specifications: TABLE & SEATS ? Shall be 8-foot long, ?- #9 expanded steel. All steel surface shall be covered with a dark brown coating on vinyl Plastisoal formulated with high impact additives and UV inhibitors. Plastisol coating shall be permanently heat-fused to the steel to an average thickness of 125 mils top and bottom coating. LEGS ? Table and bench legs frames shall be 2 3/8? O.D. steel pipe with a 2 to 3 mil ?Powder Coat? dark brown finish. Frames shall be welded into a single piece to simplify assembly and strengthen table structure. ADA ACCESSIBLE ? Shall allow for wheelchair clearances of 30? wide and 24? total knee clearance (19?+5? toe). Under table clearance shall be 27? to accommodate a nominal wheelchair seat height of 19?. TABLE STRENGTH ? Shall be guaranteed to support 500lbs/sq.ft. distributed load. Supplies shall provide documented tests and guarantee of compliance prior to shipping of the product. Tests shall demonstrate that no failure of the product will occur with an evenly distributed load that simulates snow. Material used for load testing shall not span over leg supports of tabletops and benches so that no portion of the load testing material shall be self supported. Offers must include descriptive literature on tables offered, warranty period, price and 52,212-3, Offeror Representation and Certifications filled out, DELIVERY: Delivery time is June 14, 2004. Delivery instructions: Shipping and off loading of said items at the Mirror Lake administration site (guard station) which is 32 miles northeast from Kamas, UT along Highway 150. The provisions of 52.212-1, Instructions to Offerors-Commercial; 52,212-2 Evaluation-Commercial Items, 52,212-4, Contract Terms and Conditions-Commercial Items apply to the RFQ. Evaluation Criteria to be used in FAR 52,212-2 Evaluation-Commercial items are (1) table specifications (2) past performance (including delivery time) and (3) price. Offerors must include descriptive literature on tables offered, warranty period, quanitity, price, references and phone numbers, 52.212-3, Offeror Representations and Certifications filled out. Vendors must register in the Central Contractors Registration (CCR) in order to be awarded a contract. See http://www.ccr.gov/handbook.cfm for a template of information necessary for registration. Quotes must be received by 4:30 PM March 25, 2004. Quotes can be faxed to (801) 975-3483.*****AMENDED TO ADD THE FOLLOWING: We are hereby amending the Evaluation Criteria: Evaluation Criteria to be used in FAR 52.212-2 Evaluation-Commercial Items are (1) table specifications and testing documentation (2) past performance (including delivery time) and (3) price. Bidders shall provide documented tests and guarantee of compliance. Documentation of test shall consist of photos and a written process of tests including information about the material used to simulate snow load and weight. Tests shall demonstrate that no failure of the product shall occur with an evenly distributed load that simulates snow to 500 lbs/sq. ft. Tests shall demonstrate at what weight the failure of product occurs. If the product exceeds the load of 500 lbs/sq.ft by 135% the tests may stop if there are safety concerns. Offeror shall include descriptive literature including details with dimensions of table top, benched, support and cross sections of planking on table offered, warranty period, quantity, price, references of clients who have had warranty concerns of the suppliers products and phone numbers.(By reference 52.214-21 Descriptive Literature). Also, please provide us a delivery time as to when you can deliver the tables. The information is needed by May 13, 2004 by 4:30 a.m.******
