Description

This is a combined synopsis and solicitation for the following commercial items, Item Description: Heart Start XLT basic module (includes a/c charger, data card, carrying case, ECG/defib cables and 5 year warranty), MFG: Philips Medical Systems, 2 ea. No or equal will be accepted. Item is required within 30 days ARO, and FOB destination is preferred for delivery to BLDG 418, 100 Independence Drive, Camp Douglas, WI 54618-5001. This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted) in reference to RFQ W912J2-04-T-3005. All firms or individuals responding must be registered with the Cent ral Contractor Registration (CCR). This procurement is being issued as 100% SMALL BUSINESS SET-ASIDE. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14 and Defense Acquisition Circular 91-13.
