- Scope of Work: Contractor shall furnish all equipment, labor, tools, parts, materials, tranportation and miscellaneous items necessary to remove and replace the following in the Amarillo VA Nursing Home Care Unit: ?85 each, 8? spread lavatory faucets, T&S Model 2865-04 (rigid gooseneck, blade handles). ?40 each, 4? spread lavatory faucets, T&S Model B0890 (blade handles) ?118 each wheelchair accessible offset strainer/PO tube drain assemblies (Kohler K13885 or equivalent) ?7 strainer/PO tube assemblies (Kohler K7715 or equivalent) ?125 each 17TA Chrome P Traps ?125 each sets water supply lines (hot and cold) from installed stops to the new faucets (two on each lavatory) Brand Name Only on T&S parts. Brand Name or equal on Kohler parts. Work shall be conducted during normal business hours, Mon ? Fri, 8:00am ? 4:30pm. with minimal disruption to VA patients and staff. Continuous cleaning and removal of debri shall be required. All safety precautions for the hospital must be followed at all times. All requests must be in writing. Solicitation will available on or about 5/14/2004. Bid Opening will be on or about 6/14/2004. Prior approval from COTR must be obtained for each area prior to commencement of work. Project may be delayed in the event of medical necessity. Cost of project is estimated to be between $25,000 - $100,000. SITE VISIT: May 24, 2004 @ 10:00am CST, meet in Engineering Office, Bldg T-34 NOTE: SUBJECT TO THE AVAILABILITY OF FUNDS.
