Description
1. Sources Sought Synopsis: Small Business interests shall note the Federal Acquisition Regulation (FAR) Part 19, North American Industry Classification System (NAICS) Code and Size Standard for the requirement identified in this announcement. The anticipated NAICS Code is 621111 and the size standard is $8,500,000.00. 2. The Contractor shall provide personal and non-personal services for the 42d Medical Group, Maxwell AFB, AL as outlined below: A. (1) Family Practice Physician or full time equivalent DESCRIPTION OF SERVICES. The Contractor shall provide Family Practice Physician services equal to one full-time equivalent (FTE). Contractor will provide outpatient family practice health care to Government beneficiaries at the 42d Medical Group, Maxwell AFB AL 36112, also referred to as the "military treatment facility" (MTF) herein. Contractor shall furnish all labor, management, teaching, consultations and reports. Contractor care shall cover the range of services provided in a civilian medical treatment facility of similar size, capacity and scope of operation. Performance shall be according to the requirements contained in this Performance Work Statement (PWS), and professional standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and the MTF. B. (1) Internal Medicine Physician DESCRIPTION OF SERVICES. The Contractor shall provide Internal Medicine Physician services equal to one full-time equivalent (FTE). Contractor will provide outpatient internal medicine health care to Government beneficiaries at the 42d Medical Group, Maxwell AFB AL 36112, also referred to as the "military treatment facility" (MTF) herein. Contractor shall furnish all labor, management, teaching, consultations and reports. Contractor care shall cover the range of services provided in a civilian medical treatment facility of similar size, capacity and scope of operation. Performance shall be according to the requirements contained in this Performance Work Statement (PWS), and professional standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and the MTF. C. (2) Family Practice Clinical Nurse DESCRIPTION OF SERVICES. The Contractor shall provide Family Practice Nursing services equal to two full-time equivalents (FTE). Contractor will provide outpatient family practice health care to Government beneficiaries at the 42d Medical Group, Maxwell AFB AL 36112, also referred to as the "military treatment facility" (MTF) herein. Contractor shall furnish all labor, management, teaching, consultations and reports. Contractor care shall cover the range of services provided in a civilian medical treatment facility of similar size, capacity and scope of operation. Performance shall be according to the requirements contained in this Performance Work Statement (PWS), and professional standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and the MTF. D. (7) Medical Technician DESCRIPTION OF SERVICES. The Contractor shall provide Medical Technician services equal to seven (7) full-time equivalents (FTE). Contractor will provide outpatient family practice health care to Government beneficiaries at the 42d Medical Group, Maxwell AFB AL 36112, also referred to as the "military treatment facility" (MTF) herein. Contractor shall furnish all labor, management, teaching, consultations and reports. Contractor care shall cover the range of services provided in a civilian medical treatment facility of similar size, capacity and scope of operation. Performance shall be according to the requirements contained in this Performance Work Statement (PWS), and professional standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and the MTF. E. (5) Administrative Appointment Clerk Services DESCRIPTION OF SERVICES. The Contractor shall provide Administrative Appointment Clerk services equal to five full-time equivalents (FTE). Contractor will provide outpatient family practice health care to Government beneficiaries at the 42d Medical Group, Maxwell AFB AL 36112, also referred to as the "military treatment facility" (MTF) herein. Contractor shall furnish all labor, management, teaching, consultations and reports. Contractor care shall cover the range of services provided in a civilian medical treatment facility of similar size, capacity and scope of operation. Performance shall be according to the requirements contained in this Performance Work Statement (PWS), and professional standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and the MTF. Performance shall be according to the requirements contained in this Performance Work Statement (PWS), and professional standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and the MTF. G. The Government contemplates award of a firm-fixed price contract for a base year and four one-year options. H. This requirement is to provide all services outlined above.
