Description
This is a combined synopsis/solicitation for non-personal services prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; propo sals are being requested from all sources. No formal solicitation will be issued but a statement of work follows below. This is a twelve (12) month firm fixed price acquisition. The objective is to provide the National Guard Bureau (NGB) commercial fitn ess facilities for approximately three hundred (300) members. April 6, 2004 STATEMENT OF WORK FOR JEFFERSON PLAZA 1 (JP1) COMMERCIAL FITNESS REQUIREMENT I. BACKGROUND In order to ensure the strength and stamina of Air National Guard (ANG) personnel to complete their respective missions successfully, the Department of Defense provides necessary physical fitness and training areas and facilities. Maintaining the physical fitness of ANG personnel is essential to keeping a healthy force ready to deploy in defense of the United States of America. ANG personnel displaced by the Pentagon renovation or assigned to those organizations without available fitness facilities must h ave access to fitness facilities and training areas meeting minimum Air Force standards. Military fitness standards are outlined in AFI 34-501. In accordance with AFM 34-137, Air Force Fitness and Sports Operations, and other Services Directives, fitness facilities contribute to Air Force readiness by enhancing the physical and mental stamina of military and civilian personnel. This is accomplished by providing a variety of programs that contribute to fitness and esprit de corps. II. SCOPE The contractor shall provide membership benefits to Air National Guard organizations whose official assignment is located only at JP1 eligible for coverage under this formal contract. Members temporary, permanently assigned or working at Andrews AFB, MD a re not eligible. Eligible participants are defined as all active duty and civilian personnel who are assigned to organizations that are funded through Operating Agency Code (OAC) 41 and work outside of the Pentagon in Crystal City. Organizations include: NGB/CF, NGB/XO, NGB/DP, NGB/FM, NGB/C4, NGB/XP, and NGB/CIO. III. MINIMUM REQUIREMENTS OF THE PHYSICAL FITNESS FACILITY In order to support the Air National Guard mission effectively, the contractor fitness staff must be knowledgeable of preventative health maintenance and weight management. The contractor facility must offer core fitness programs that contribute to cardio vascular fitness, strength conditioning and flexibility, i.e. total fitness. As a minimum the following fitness programs/equipment must be offered: cardiovascular equipment, strength conditioning equipment, aerobic classes and walking and jogging program s. The commercial fitness facility shall be located within 1 mile of JP1 (1411 Jefferson Davis Highway in Arlington, VA). IV. FULL MEMBERSHIP BENEFITS A. Membership in the fitness facility shall include: 1. All standard commercial services offered by the fitness facility, 2. Unlimited access during normal operating hours, and 3. Additional discounts normally extended to members. B. Membership does not include: 1. The use of health spas, massages, racquet clubs, and golf courses, bowling centers, sporting clubs, and swimming facilities unless included in a standard commercial membership. C. The rights to guest passes and membership privileges shall belong to the Air National Guard as represented by the Program Manager, and not to the individual participants. V. MEMBERSHIP CARDS Membership cards issued by the contractor shall identify the name of the government participant along with his or her unit. Any replacement fee for lost cards is the responsibility of the participant. VI. PROCEDURES A. Memberships shall run from the first day of the month for a 12-month p eriod. The member will register through Air National Guard Web Site https://airguard.ang.af.mil/svx. Each customer will review the membership rules and submit application through the web site NLT 25th of each month to be eligible for the following month. A membership approval pass will be printed through the web site and Air National Guard (ANG) Fitness Program Manager will issue commercial membership cards given to them by the commercial fitness manager. B. Once the member has registered and is approved; the ANG Fitness Program Manager will issue membership cards on the last working day of the month between 0900-1100 and 1300-1500 at the commercial fitness facility. The fitness facility shall submit a mo nthly member??????s usage report NLT five days from the end of that month to the Fitness Program Manager. If the member elects to initiate membership prior to the first of the next month without approval from the Fitness Program Manager, then the member s hall be responsible for any fee charged. C. In addition to memberships for ADSW and TDY personnel the STAT tour supervisor must complete the enrollment process two days prior to the start dates. Once supervisor completes enrollment they must brief customer on rules and policies of the program. A membership pass approval slip will print for the selected time period for day, week, or month. The customer uses this membership pass approval to use the commercial fitness facility. Day is a 24-hour period, week is a 7-day period and month is 30-day period. Pass must be used in consecutive order with dates approved by supervisor. STAT tour supervisor must register through the Air National Guard Web Site https://airguard.ang.af.mil/svx. ADSW and TDY personnel must follow the same rules and applicat ion process to be granted a temporary membership card. The fitness facility shall submit an invoice to the Fitness Program Manager monthly along with ADSW and TDY member usage report. Invoices for ADSW and TDY personnel must be identified in a monthly in voice with a listing of members?????? names. The Government shall not be responsible for making a payment until the Fitness Program Manager receives a completed list by name with the invoice of services. D. Stay-in-School personnel are authorized to use the commercial fitness program if they are 18 years of age. The STAT tour supervisor completes the same process for ADSW and TDY personnel. The fitness facility shall submit an invoice to the Fitness Pro gram Manager monthly along the ADSW and TDY member usage report. Invoices for Stay-in-School personnel must be identified in a monthly invoice with a listing of members?????? names. The Government shall not be responsible for making a payment until the F itness Program Manager receives a completed list by name with the invoice of services. E. The fitness facility shall submit an invoice for 300 membership cards for a one time bill based on a 12-month period. The Fitness Program Manager will manage the membership cards. If less than 300 people choose to continue membership during a given m onth, those memberships will be placed on in-active status. Additional membership may be granted. The fitness facility will need to submit an invoice for additional members beyond the first 300. F. Participant memberships may be terminated upon 30 days notice with termination effective at the end of the following full-calendar month. Participants who move back to the Pentagon, separate from the Air Force, or are subject to a Permanent Change of S tation (PCS) or Permanent Change of Assignment (PCA), must provide supporting documentation to the Fitness Program Manager in order to terminate their membership as soon as possible. This includes JP1 members re-assigning to Andrews AFB, MD or other military installations where fitness facilities are available. Additional funds due the Government resulting from membership termination shall be credited with in-active stat us of membership card. Anyone able to provide the aforementioned services is encouraged to respond. If you are selected, you must be registered in the Central Contractor Registration (CCR) (http://www.ccr.gov/) in order to receive the award. EVALUATION PROCESS FOR AWARD: Award will be made to the technically acceptable, lowest priced quotation from a responsible Quoter as defined in FAR Part 9, who also has satisfactory record of past performance. Evaluation Factors are as follows: Technica l Approach. Request you prepare a Technical Approach that includes information that demonstrates your technical capability and proven past performance to successfully complete the requirement as outlined in SOW Section III. (Max pages 5) Past Performance: submit three (3) references of similar work performed in the past three years. References should include contract numbers, performance periods, and point of contact with phone numbers. Past Performance elements to be reviewed include quality, delivery, cost control, and customer service. Execution Plan: Illustrate or discuss the project execution, example the issue of membership cards, an updated computer system capable of maintaining separate membership i.e. national guard member, ADSW, TDY and pri nting a report monthly. (Max pages 5). Price: submit a firm fixed price quote for the effort outlined in the Statement of Work. Price should be clearly stated for full month, weekly and daily and/or annual membership. The solicitation document and incorporated provisions and clauses are those in effect through Fe deral Acquisition Circular 01-21.
