- A site visit was held at the MO-AVCRAD Shop in Springfield, MO on May 11, 2004. The following are changes and/or additions as a result of that meeting. 14a) Offers are to be broken down in the following format: a) Equipment/material b) Labor hours for installation c) Labor hours for training 14b) Information to include with your offer: As stated in the original solicitation, all contractors were reminded to submit the following with their offer: (1) Submit completed Form FAR 52.212-3 Offeror Representations and Certifications (2) Shop Drawings and (3) At least three past performance work references, preferably downdraft bench projects. References to include individualâs name, company, phone number, type of work and date completed. Also, as solicitation states, offerors are to follow the format, or their offer could be found unacceptable. Provide name, address and phone number of authorized servicing company with your offer. 14c) Change the electrical power to: 480Y/277 volt, 3 phase, 4 wire, 60 hertz (request vendor provide shop drawings to include identifying amperage ratings under full load and during starting conditions). The shop drawings can be in CAD format or hard copy format. (This was originally listed as an option --- the 480Y/277 is now to be included in the primary offer) Approved shop drawings from the successful contractor will be returned within 5 business days from award date. If unsuccessful contractors want their drawings returned, request must be submitted by email to Shirley.zbinden@mo.ngb.army.mil and the drawings will be returned within 5 business days upon receipt of request. 14d) Do not offer electrical power of 208Y/120 volt. 14e) Offer is to include a multi-stage filtration system, which will allow air to be returned to the occupied space. This system will include, at a minimum, pre filters, carbon filters, cartridge filters and HEPA filters (Amendment #0003 states high e fficiency 99.999% on .5 micron dust particles) 14f) Offer is to include a dust particle alarm, which shuts down the fan system and sounds an alarm. 14g) Offer is to include two (2) O&M Manuals. 14h) Information: Wendy of Environmental Department provided a listing of testing results on material that comes off of the blades. This was provided to all contractors attending the site visit. 14i) In the original solicitation statement of âEvaluation of offers ---- Technical and past performance when combined are equal to price ------- CHANGE TO: Technical and past performance when combined are more significant than price. 14j) No other changes. Offer due date of June 1, 2004 remains the same. NOTE: THIS NOTICE WAS NOT POSTED TO WWW.FEDBIZOPPS.GOV ON THE DATE INDICATED IN THE NOTICE ITSELF (14-MAY-2004); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
