Description

Provide A/E servcies for design of project to "remodel Endoscopy and Hemodialysis" at the VA Medical Center, 800 Zorn Ave., Louisville KY. Remodel approximately 18,220 GSF for Endoscopy, Hemodialysis, Neurology, Sleep Lab, Pulmonary Medicine, Respiratory Therapy, and adjacent space on the 5th floor. Design includes the preparation of complete drawings in AutoCAD 2004 format, specifications, and construction cost estimates per VA guidelines. Project involves major renovation of most areas and minor finish improvements in others. The new design will conform to the current standards and provide adequate facilities for our patients and visitors. Design is also to include preparation of detailed phasing plan/sequence of operation for minimum disruption of hospital operations. The A/E team must include a certified industrial hygienist for planning of asbestos abatement throught the project. Estimated construction cost is between $2.5 and $3.5 Million. Completion time is 180 calendar days after notice to proceed is signed. NAICS Code 541310 applies. Interested/qualified firms must submit (in triplicate)SF 254 & 255 per Numbered Note 24 to the contracting Officer, no later than June 30, 2004 @ 3:30PM local (EST) time. A/E firms must be located within a 30 mile radius of the VA Medical Center, Louisville KY to be considered. No construction contract may be awarded to the firm that designs the project. Contract award is subject to availability of funding.
