Description
1. The Contractor shall provide professional social work, discharge planning and case management activities for Indian beneficiaries of Gallup Indian Medical Center, Gallup, NM. 2. The Contractor shall work that will require direct patient intervention, assessment, planning, coordinating and monitoring services with the Federal, State, Tribal, and other local service providers. 3. The Contractor shall exercise mature, professional judgment and flexibility in handling a wide range of case work skills in the study, diagnosis, and treatment of patients and their families who present an unlimited range of difficult health related psycho-social, mental health, environmental and other problems. 4. The Contractor shall determine independent conclusions of isolated and valnerable patients who have complicated and conflicting needs regarding medical conditions/decisions, treatment plans, resources and relationships. 5. The Contractor shall provide integrated treatment plans and furnishes continuing intervention services to patients and families during the critical and adjustment phases of illness and disabilities and overcoming dependency needs. 6. The Contractor shall use culturally relevant interventions to insure recognition of cultural values/philosophy of health care receipients. 7. The Contractor shall conduct initial assessments, develop individualized service plans that will identify programs and services appropriate to meet patients' needs. 8. the Contractor shall prepare and interprets case studies and histories of patients for physicians, health care teams and community social and other agencies for their use in reaching decisions for further medical or psycho-social treatment. 9. The Contractor shall prepare patient referrals and coordinates services for patients with other professionals, community agencies/resoures such as social service, education, law, judicial and health organizations. 10. The Contractor shall make independent professional decisions and recommendations for patient care planning regarding hospital discharge of patients to home or alternate living, community based or contract care rehabilitation or long term care. The changes on the lives of patients' families and assist through supportive counseling and therapy with decision making. 11. The contractor shall demonstrate the knowledge and skills, principals of growth and development over the life span, posses to asess data reflective of the patient's status; and requirements relative to age-specific needs of the neonate, pediatric, adolescent, adult and geriatric. 12. The Contractor to comply with Gallup Service Unit policies and procedures.
