Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is FA3300-04-T-0023, and is issued as a Request for Quote using Simplified Acquisition Procedures under the test program for certain commercial items found at FAR Subpart 13.5. This requirement is for the purchase of : CLIN 0001 – Helmets, parade, chrome plated, molded plastic with adjustable headband and white nylon adjustable chin strap, Quantity, 500 ea. CLIN: 0002 – White Cord, shoulder, bright polycord, standard size with wrapped pin attached, Quantity: 5000 each. Delivery date is May 28, 2004. Ship entire quantity to 500 Maxwell Ave, Maxwell AFB, AL, 36112. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-22 and the Defense Federal Acquisition Regulations (DFARS) change notice March 23, 2003. It is the contractor’s responsibility to be familiar with applicable clauses and provisions. This acquisition is being issued on an unrestricted basis. The North American Industrial Classification System Code (NAICS) is 339113 with a small business size standard of 500 employees. The following clauses and provisions are incorporated by either reference or full text and are to remain in full force in any resultant purchase order; FAR 52.212-1, Instructions to Offerors, Commercial Item (Jan 2004) applies to this acquisition. Submit proposals via email or facsimile to the information below. The Government will make an award to the lowest priced, responsible, contractor whose proposal conforms to the solicitation. The contractor must be registered in the Central Contractor Registration database and in Wide Area Work Flow before an award can be made. Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Jan 2004), DFARS 252.212-7000, Offeror Representations and Certifications Commercial Items (Nov 1995), and DFARS 252.225.7000, Buy American Act Balance of Payments Program Certificate (Apr 2003) with the proposal. Clauses and provisions may be accessed via the internet, http://farsite.hill.af.mil. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Oct 2003), is incorporated into this RFQ; FAR 52.204-7, Central Contract Registration (Oct 2003); DFARS 252.204-7004 Alt A, (Nov 2003). FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Apr 2004) is applicable. The following additional FAR clauses under paragraph (b) are applicable: 52.222-21, Prohibition of Segregated Facilities (Feb 1999); 52.222-26, Equal Opportunity (Apr 2002); 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era (Dec 2001); 52.222-36, Affirmative Action for Workers With Disabilities (Jun 1998); 52.222-37, Employment Report on Disabled Veterans and Veterans of the Vietnam Era (Dec 2001); 52.225-13, Restrictions on Certain Foreign Purchases (Dec 2003); 52.232-33, Payment by Electronic Funds Transfer Central Contractor Registration (Oct 2003); 52.233-3, Protest After Award (Aug 1996). DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items (Jan 2004) applies to this acquisition and specifically the following additional DFAR clauses under paragraph (b) are applicable: 252.225-7001, Buy American Act and Balance of Payments Program (Apr 2003); 252.232-7003, Electronic Submission of Payment Requests (Jan 2004).
