Description

This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6, and supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Qu otes are requested; however, a written solicitation will not be issued. Solicitation number W911S8-04-T-0037 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Fed eral Acquisition Circular 2001-15. The requirement is small business set-aside with a NAICS Code of 332312, and size standard of 500. The description of the commercial items is as follows: Clin 0001: Fabricated Metal Door, Formalloy 400, 1/4 inch thick, AR 400 plate doors with holes and slots, Type 1, Back Door, per sample; Quantity: 160 each. Clin 0002: Fabricated Metal Door, Formalloy 400, 1/4 inch thick, AR400 plate doors with holes and slots, Type 2, Front Door, per sample; Quantity: 160 each. Please fax or e-mail to request copy of 2-page sample drawing. Sample will be sent by fax only. State if FOB is Destination or Origin. If Origin, state shipping charges. Delivery is to be as soon as possible to DOL, Bldg 9580, Fort Lewis, WA 98433-9500. Plea se state Estimated Delivery Date. Any award resulting from this request for quotations will be made to a responsive, responsible quoter using a SF1449 form. Quotes will be evaluated on a lowest price technically acceptable competitive acquisition evaluat ion factor whose offer conforms to the requirements and who has acceptable past performance. A sample must be submitted prior to award. Award will be made on an all or none basis. If quotes are provided for an equal item, specifications must be submitte d. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. An addendum to this provision is applicable, in which the term offeror (whenever it appears) shall be replaced with the term quoter. Quoters are required to su bmit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes. FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. Far clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.212-5(b) are applicable to this acquisition: 52.222-3, 52. 222-19, 52.222.21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33. The following clause within DFARS clause 252.212-7001(b) is applicable to this acquisition: 252.225-7001.
