Description
WORK-STATEMENT A. Subject: Allied Health Technician II B. Place of Performance: Health Services Unit, Federal Correctional Institution Yazoo City, Mississippi to include the Magnolia Camp. The contractor will sign the consultant log, indicating the date and time of arrival and departure to the institution. C. Schedule: A session is defined as 2 hours. A work day will normally consist of 4 sessions. A contractor will normally work five days a week (Monday-Friday) for total not to exceed 15 sessions per week. The contractor will normally work from 07:30 a.m. to 2:00 p.m. (Monday - Friday). However, the hours may be adjusted to meet the needs of the institution. In addition, a 30 minute lunch period shall be taken between the hours of 10:30 a.m. and 1:00 p.m. The lunch period shall be taken as not to interrupt procedures being performed and at the discretion of the COTR. The above schedule is subject to some variation as the needs of the institution may vary. A schedule that is mutually acceptable to the government and the contractor can be determined after award of the contract. The contractor may not work more than thirty (30) hours per week. The contractor shall, within fourteen (14) days of award, submit a formal request to the COTR, with approval required by the Health Systems Administrator (HSA), for time-off not to exceed 15 sessions (a file copy shall be forwarded to the Contracting Officer). In addition, for any unscheduled absences greater than 8 sessions, the contractor will be subject to provide a certified, pre-approved replacement at the contractor?s expense. Failure to comply with this or other requirements of the contract, may constitute cancellation of the contract. D. Supplies: Will be furnished by the Health Services Unit. E. Scope of Service: The contractor shall be responsible for the following tasks: (Note: Some duties may not be required on a daily basis. Responsibilities within the scope of work may vary from day to day and week to week.) 1. Filing of Medical Records in terminal digit order. Provide medical practitioners with the Medical Records for daily sick call and speciality clinics. 2. A working knowledge of various medical reports and the ability to file lab, x-ray, consultant and other reports in the proper section of the medical records. 3. Scheduling patients for medical appointments both in-house and in the local community for special consultation. Prepare medical town trip forms and ensure completeness for routing for signature and approval. 4. Maintain confidentiality of medical records in accordance with Privacy Act of 1974 and Bureau of Prisons regulations. 5. Review medical records for medical and legal completeness and provide documentation of deficiencies and corrective action taken. 6. Maintain medical records and file reports as needed. 7. Schedule inmates for local diagnostic studies and speciality medical services. Routinely retrieve and enter information regarding call-outs and appointments. 8. Other administrative tasks as required by Health Systems Administrator (HSA), Asst. HSA, physicians and practitioners. 9. Participate in Total Quality Management as a committee member. 10. Provide written responses to Inmate Requests to Staff Member relating to medical record inquiries and conduct inmate Open House providing information from medical records in accordance with BOP policy. 11. Enter inmates on call-out system for various department needs. 12. Enter sensitive medical date (SMD) on a daily basis ensuring all data is complete. 13. Process and track newly admitted (A&O) inmates for medical clearance to ensure compliance with BOP policy. 14. Enter all inmate duty restrictions into Sentry on a daily basis. 15. Initiate and processes other medical record forms as deemed necessary. 16. Participates in Improving Organizational Performance (IOP) by collecting and aggregating data. Specific areas of study within Radiology will be assigned by the Assistant Health Systems Administrator. 17. Prolonged standing and lifting, and climbing stairs will be required. 18. Must possess typing and basic computer skills. 19. Track and schedule outside contract physician patients. 20. Process, order and receive inmate eyeglasses. F. Management: 1. The contractor will provide a monthly billing statement to the Health Services Administrator that accurately reflects the days of each visit, the number of hours or sessions of each visit and the total dollar amount due for that months services. Billing will be in 14-day increments only. 2. Training in some areas of computer programs will be provided as needed. G. Institution Security: Will be cleared for work by standard Bureau of Prison policy. The contractor agrees to adhere to all regulations prescribed by the institution for safety, custody, and conduct of inmates. The offeror shall attend an orientation program prior to assuming institution involvement. Reimbursement to the contractor for time spent in institution orientation or other specific security related training will be allowed at the pro-rated session rate. H. Anticipated Period of Performance: Date of award of contract July 1, 2002 through September 30, 2003, pending the availability of funds. Should funds not be immediately available, contract award for the entire fiscal year will be postponed until such time as funds become available. Base Year = Date of Award - Option 1 = July 1, 2003 - I. Special Qualifications Requirements: 1. Completion of training with a minimum of two years experience. 2. Contractor must provide proof of past experience or academic achievements in a training program applicable to the Scope of Service in reference to this work statement. In addition, prior training in this specialized field is desired. 3. Prospective employee(s) cannot have had a negative employment termination within 36 months prior to initiating vouchering process on them. 4. Prospective employee(s) must be employed by contractor (or be self employed as the contractor) at the time their name(s) are submitted for the vouchering process. 5. Contractor shall ensure submitted prospective employee(s) meet requirements of vouchering process as described in the Civilian Personnel Manual 3000.02. 6. Prospective employee must successfully pass a National Agency Check and Inquiries(NACI). 7. Contractor shall be allowed to submit up to a maximum of 3 prospective employee(s)names per contract at the time that the bid package is submitted. Failure of said employee(s) to meet any of the above criteria will result in the awarding of the contract to the next lowest bidder. All perspective employees are subject to interview prior to award. 8. Contractors shall wear appropriate professional attire at all times. Jeans (of any color), T-shirts, sandals (of any type), tank tops, and sweat pants/shirts are not considered appropriate professional attire. 9. Contractor will be required to lift (no greater than 30 lbs.) and stand for prolonged periods of time. Contractor will also be required to climb stairs.
