- Description
- The purpose of this amendment is to include the Statement of Work with the combined synopsis/solicitation for Youth Soccer Camp Instructor. STATEMENT OF WORK SUMMER SOCCER PROGRAM The contractor will provide for non-personal services to staff and operate a weekly soccer program for DoD youth ages 6-18, which will be paid for by government funds. Required services are specified within: A. Summer Soccer Program will travel to 13 USAFE installations, between 14 June – 27 Aug 04. Installations to be visited are as follows: RAF Croughton, Mildenhall, Fairford, Alconbury and Lakenheath in the UK, Spangdahlem, Geilenkirchen, Ramstein, and Rhein Main AB’s in Germany, Aviano AB in Italy, Moron AB in Spain, Incirlik in Turkey, and Lajes Field in the Azores. Contractor is responsible for all travel costs within Europe. If the contract is awarded to a CONUS firm, local youth programs will provide the ground transportation while in the while in each separate country. USAFE/SVPCY will coordinate specific dates for each camp once contract has been awarded. B. Each program will consists of 5 days, 2 sessions of 3 hours per day C. Child to staff ratio for the contractor will be 15 to 1 D. Coaches will be certified by national soccer body, be fluent in the English language and will have received training in conducting programs for youth E. Each program participant will receive 15 hours of instruction, a participation shirt and a camper evaluation F. Each week the program will conclude with an awards presentation for he participants and parents G. Each base would receive a donation of 30 soccer balls for use at the program and to be utilized by the base after close of the program H. Each base will receive 6 hours of parents’ and/or coaches clinics to be held over 2 evenings during the week at no additional charge I. Coaches will be trained and certified in First Aid J. Base programs will provide a soccer pitch and goals for the program. K. Contractor is responsible for all other equipment and supplies. L. The local youth program will provide lodging and register all program participants. M. Local programs will provide lodging for contractor. N. Contractors are responsible for all meals. O. HQ USAFE/CONS will provide each individual instructor a Letter Of Identification for access to military installation and activities. Contractor is required to provide personal data as requested a minimum of 10 calendar days before travel starts.
