- 42 CONS/LGCA Maxwell AFB intends to award a sole source contract to Humana Military Healthcare Services, 500 West Main Street, P.O. Box 740062, Louisville, KY 40201-7462 to provide continuous healthcare services to the 42d Medical Support Group at Maxwell AFB, AL. from 01 Aug 04- 30 Nov 04. Authority: 10 U.S.C. 2304 (C) (1), Only one responsible source and no other supplies or services will satisfy agency requirements. The objective of this effort will be the delivery of the following: PERSONAL SERVICES: Provide (1) Family Practice Physician, (1) Internal Medicine Physician and (2) Family Practice Nurses for the Maxwell Ambulatory Healthcare Center. NON-PERSONAL SERVICE: Provide (5) Administrative Medical Appointment Clerks, (7) LPNs, (5) Administrative Medical Clerks and (3) Medical Laboratory Tech/Phlebotomist Services for the Maxwell Ambulatory Healthcare Center. The agency will require all qualified contract personnel go through an in-depth credentialing process which can take up to 45 calendar days. The following will be required: Initial applications for clinical privileges, to include credentials action history, malpractice history, and completed copies of the below listed documents, shall be submitted to the Military Treatment Facility (MTF) Credentials Committee 30 calendar days from receipt of notice of contract award, or when requested by the QAP. Additionally, the Contractor shall make all proposed health care providers available for interview by the MTF during the credentialing process. a. Copy of valid, current, unrestricted state medical license and applicable Board Certification. b. Provider education and training. c. Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification. d. Current certification in Neonatal Resuscitation Program (NRP), if applicable. e. A signed consent for release of information. f. Copy of Drug Enforcement Agency (DEA) certificate, if applicable. g. A copy of the proposed contract provider?s professional resume, accompanied by that individual?s sworn affidavit of the truthfulness of same, indicating experience, training, and technical expertise in the type of care to be rendered. h. A list of all states in which contract provider currently holds or has held a license to practice related services. i. Certification of current physical examination for each employee. The certification shall contain a signed statement by the examining physician stating that the employee is free of any contagious diseases. Physical examination must be current as of 12 months prior to application for privileges. The Contractor is required to begin full performance of the contract requirements on 01 August 2004 The Contractor must submit only complete credentials applications for processing. Incomplete applications (those that do not fully comply with the requirements) will not be accepted by the Government and will be returned without action. The Contractor will submit applications for only those health care providers who can reasonably be anticipated to render the actual, substantial performance on the contract. The submission of primary health care providers for credentialing who lack the requisite qualifications, or who will not work at all, is unacceptable and will be considered a failure to perform. PERSONAL SERVICES LICENSE/REGISTRATION: The health care provider shall possess a valid unrestricted license to practice Internal Medicine from the state of Alabama. The health care provider shall possess a valid unrestricted license to practice Family Practice Medicine from the state of Alabama. The health care provider shall possess a valid unrestricted license to practice Nursing from the state of Alabama. All contract personnel must be available and ready to begin work 01 Aug 04 to avoid any interruption of medical services. Interested parties should be able to perform all the services listed under personal and non-personal services and may identify their interest and capability to respond to the requirements not later than 04 Jun 04.
