Description

Contractor to provide Scarce Medical Specialist Services of a certified technician to perform Electroencephalographic studies (EEG'S) utilizing contractor provided equipment. Proposed price to include professional component. Services to be provided to and at the Department of Veterans Affairs Medical Center, 500 West Fort Street, Boise, ID 83702. Contract will be for the period of July 1, 2004 through June 30, 2005 with four pre-priced option years to renew. This procurement is totally set aside for small business, NAICS Code 621111. Web based solicitation documents are available or may be requested in writing from linda.liquin@med.va.gov, in writing via FAX at (208) 422-1139. All written requests must be on company letterhead with point of contact, address, telephone and FAX number.
