Description

**THIS AMENDMENT TO THE SYNOPSIS/SOLICITATION IS ISSUED TO CORRECT THE NAIC CODE, TYPOGRAPHICAL AND FORMATTING ERRORS. ALL ELSE REMAINS THE SAME.** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation is issued as a Request for Quotation (RFQ). The solicitation and incorporated provisions are those in effect through Federal Acquisition Circular 2003-23. This procurement is a Total Small Business set-aside. NAICS code is 423390. Small business size standard is 100 employees. Requirements are as listed: SUPPLIES AND PRICES: (0001) Corrugated Polyethylene Culvert - Dual Wall (Corrugated Exterior, Smooth Interior); 24-inch Diameter, 20 Foot Length, Estimated Quantity - 1000 Each; (0002) Corrugated Polyethylene Culvert - Dual Wall (Corrugated Exterior, Smooth Interior); 36-inch Diameter, 20 Foot Length, Estimated Quantity - 1000 Each; (0003) Corrugated Polyethylene Culvert Coupling Bands, 24-inch Diameter, Estimated Quantity - 25 Each; and (0004) Corrugated Polyethylene Culvert Coupling Bands, 36-inch Diameter, Estimated Quantity - 25 Each. Actual quantities ordered will depend upon prices offered and funds available. DESCRIPTION: The work to be done under these specifications consists of furnishing and delivery of the supplies listed as specified herein. DELIVERY: Items 0001 - 0004 to be supplied under this purchase order shall be delivered F.O.B. Destination to the North Bend Ranger Station, 42404 S.E. North Bend Way, North Bend, Washington 98045. Delivery shall be by a truck with a flatbed or flatbed trailer (enclosed conveyance is not acceptable). Delivery shall be within 30 days of Contractor receiving purchase order. Items shall be unloaded in the back of the compound as directed by the Forest Service representative. Contractor is responsible for unloading delivered items. Prior to shipment, the Contractor shall provide 48 hours advance notice of delivery date to the Government by contacting: William Strong, (360) 888-1421, ext. 259. MEASUREMENT AND PAYMENT: Payment will be made at the purchase order unit price for the quantity of acceptable material delivered. No payment shall be made for supplies damaged because of Contractor fault or negligence. INVOICES: Invoice shall be sent to the following address: Acquisition Management, Olympica National Forest, 1835 Black Lake Blvd SW, Ste A, Olympia, WA 98512-5623. The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. The provision FAR 52.212-2, Evaluation - Commercial Items, will be used. OFFERS WILL BE EVALUATED ON PRICE AND PAST PERFORMANCE. THE TWO FACTORS ARE OF EQUAL IMPORTANCE. OFFERORS SHOULD COMPLETE AND INCLUDE A COPY OF THE PROVISION FAR 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS COMMERCIAL ITEMS, WITH THE OFFER. Clause 52.212-4, Contract Terms and Conditions - Commercial Items, and Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, apply to this acquisition.
