Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The North American Industry Classification System (NAICS) code applicable to this solicitation is 423450, Medical, Dental and hospital equipment and supplies wholesalers, and the small business size standard is 100 employees. This is a full and open competition (unrestricted). The VA healthcare Network Upstate New York at Syracuse intends to purchase one (1) Lumenis Opal Photoactivator system, (#0637-602-01) with a Haag-Streit English Optholamic Laser Link Opal Photoactivator laser for Photo Dynamic Terapy Treatment (# 0637-602-01), one (1) wallmount for the Opal Laser, (0621-354-01), one (1) each, pair safety spectacles, footswitch, operator manual, laser safety sign and in-service. Price offered must include all items. Salient Characteristics for the Lumenis Opal Photoactivator include a unique feature that allows measuring the power output to assure the visudyne drug is sufficiently activated by the laser and a homogenizer that prevents "hotspots" (areas of overtreatment) in the laser treatment. Contractor is responsible for all freight charges. This solicitation document incorporates Federal Acquisition Regulation (FAR) provisions and clauses in effect, and VA Acquistition Regulation (VAAR) clauses as follows: 52-211-6 Brand Name or Equal. The provision at 52.212-1, Instruction to Offeror--Commercial Items, applies to this solicitation. The provision at 52.212-2, Evaluation _ Commercial Items, applies to this solicitation. Paragraph (a) of FAR 52.212-2 is added to read: Best Value: Technical capability, past performance, and price. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/.
