- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offerors are being requested and a written solicitation will not be issued. Solicitation # W912R1-04-T-0113 is issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Cir cular 2001-23. This solicitation is unrestricted. It is advertised with a North American Industry Classification Code (NAICS) 337910. The government intends to award a firm fixed price contract. A brief synopsis of the required item is as follows: Quee n Size Mattress with personalized settings from soft to firm improve sleep and relieve back pain, remote control led dual independent adjustability, 20 year warranty or more, mattress with unique patented air chambers that adjust to your body, pillow top a dded to the mattress, border walls to create better edge support, patented I-Beam construction for superior support, center foam in the mattress, bed systems delivered, set-up and the old mattresses removed. The quantity required is 9 Each which includes t he bed system, mattresses and box springs. The following clauses and provisions apply to this acquisition: FAR 52.212-1 Instruction to Offerors Commercial Items; FAR provision 52.212-3 Offerors Representations and Certifications Commercial Items shall be completed and submitted with the offer; FAR 52.212-4 Contract Terms and Conditions - Commercial Items; FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items with the following applicable clauses in p aragraph (b), 11,12,13,14,15,16,24, and 27. The following clauses apply in section (c), 1 and 2. FAR 52.252-1 Solicitation Provisions Incorporated by Reference (This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as is they were given in full text); FAR 52.252-2 Clauses Incorporated by Reference (This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text); DFAR 252.204-7004 Requir ed Central Contractor Registration; DFAR 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders. *Note: The full text of a clause/provision may be accessed electronically at this address: http://farsite.hill.af.mil. Award will be made to the offeror, which is most advantageous to the government. The following areas will be evaluated: Price.
