Description

(I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) Solicitation FA487704Q0321 is being issued as a request for proposal. (III) This solicitation incorporates clauses and provisions in effect through the Federal Acquisition Circular 01-07. (IV) This requirement is being synopsized as a small business set-aside under standard industrial classification code 3272, NAICS Code 327390, small business size standard of 500 employees. (V) COMMERCIAL ITEM DESCRIPTION: (3 each) Precast Concrete Buildings. Building size shoud be 12? x 20? x 9? using Split Face Block. Each building should have 1 each 6? x 6?8? 18ga steel door with deadbolt, drip cap and crash chain. Include louver ?ventilation? system, interior lights, exterior lights (one near door), and elctrical panel/breaker system. Power will also need to be installed/connected to 2 of the buildings. All buildings will be placed onto site prepared by the customer. Unit prices should include transportation and off-loading costs. (VII) Place of delivery: 355 EMS, 3820 S. First Street, Bldg 1440, Davis-Monthan AFB, AZ 85707. F.O.B. POINT IS DESTINATION. (VIII) Provision 52.212-1, Instructions to Offerors- Commercial Items applies. (IX) Provision 52-212-2, Evaluation _ Commercial Items applies. Proposal will be evaluated on price, price related factors and past performance. (X) Offerors are not required to submit a completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items with their offer. (XI) Clause 52.212-4, Contract Terms and Conditions- Commercial Items and any addenda the clause applies to this acquisition. (XII) Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to the award document. (XIII) N/A. (XIV) N/A. (XV) N/A. (XVI) Proposals will be due to the 355th Contracting Squadron, 3180 S. Craycroft Rd. Davis-Monthan AFB, AZ 85707, by 23 June 2004, 12 p.m. local time.
