Description
The Social Security Administration (SSA) intends to award a sole source contract for a special study to the International Institute for Healthcare and Human Development, 6310 San Vicente Blvd., Suite 240, Los Angeles, California 90048 in response to an unsolicited proposal. The proposal demonstrates a novel systems approach that focuses on the implementation of a comprehensive and coordinated service delivery model facilitating healthcare, community and vocational services for both SSDI and SSI beneficiaries, primarily those with disabilities related to chronic disease. The study will focus on whether the proposed integrated approach facilitates and sustains gainful employment. This notice is for informational purposes only and is neither a request for quotation nor an announcement of a forthcoming solicitation. Interested parties that would like to provide comments can submit them to Christine Nagengast via e-mail at Christine.Nagengast@SSA.gov by 3:30 p.m. EST on or before Monday, July 12, 2004. The Government does not intend to pay for information solicited or received. Respondents will not be notified of the results of the evaluation of information received. The Government will consider all information from responsible sources, but a determination by the Government not to compete this action is solely at its discretion.
