Description
This is a combined synopsis/solicitation for a commercial service, prepared in accordance with the format in FAR Subpart 12.6, and supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are requested; however, a written solicitation will not be issued. Solicitation number W911S8-04-T-0047 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-03. The requirement is small business set-aside with a NAICS Code of 333911, SIC Code 4610, and size standard of 5 mil. The description of the commercial service is as follows: Statement of Work SPECFICATIONS FOR Test, Certify all Backflow Preventors 1. SCOPE OF WORK ??????Backflow Testing, Repair and Re-Certification. Provide all equipment and personnel to Test, and certify all Backflow Preventors at Fort Lewis, Washington. See Technical Appendix #1 for list of buildings. 1. SCOPE OF WORK. Provide all equipment and personnel to Test, and certify all Backflow Preventors at Fort Lewis, Washington. See Technical Exhibit #1 for list of building. 2. Provide a schedule of work for approval. 3. Data program must be compatible with Maximo/Microsoft Access latest version. Reports shall be in the format compatible with the Washington State Requirements. 4. A one line drawing consisting of location within building structures of Backflow Preventors will be in exhibit #2. 5. The contractor shall be familiar with, and incorporate into this work, the requirements outlined in the Fort Lewis Environmental Policy and the Public Works Environmental Management System (EMS). The EMS is based on the ISO 14000 standard (Environmenta l Management System ?????? specification with guidance for use) and its purpose is to ensure that all activities, procedures, and processes on the installation conform to the Fort Lewis Environmental Program. The contractor shall incorporate procedures to ensure that the work does not produce negative impacts to the environment and ensure that all submittals are produced using recycled paper and electronically. 6. HOURS OF OPERATION. Normal working hours is from 0700 to 1600 Monday through Friday unless previously arranged and approved by the contracting officer. Work being performed at Fort Lewis will required that the contractor work other than normal working hours as stated above. The contractor may request, in writing to the Contracting Officer, to work other than normal hours. 7. Highway vehicles owned or leased by contractors shall be Furnished with identifying markings reflecting, minimally, the contractor's name, home city and local phone number. 8. Contractor??????s workmen shall have on either a uniform with the Firm name and the workman??????s last name or shall have a badge Pinned on with both the firm name and the workman??????s photograph and full name. 9. A sixty-day time period is allotted for completion of this contract; contract begins on the award date signed by the contracting officer. 10. All Backflow devices that need repair, replacement and are Not to code will be identified, and describe defiencys in the report. A failure rate of 15% of devices is expected. This usually consists of cleaning and re-testing of backflow devices. Backflows that require new kits will be in the requirements part of the bid schedule. Award will be made on an all or none basis. Provision 52.212-1, Instructions to Offertory-Commercial Service, applies to this acquisition. An addendum to this provision is applicable, in which the term offer or (whenever it appears) shall be replaced wit h the term quoter. Quoter are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes. Also, please provide Federal Tax ID Number FAR clause 52.212-4, Cont ract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. Far clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition.
