Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, F7442741750500, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-24 and Defense DCN 04-0625. If two (2) or more responsible and technically acceptable small businesses respond, this acquisition will be 100% set-aside for small business, if not, it will be unrestricted. The Federal Supply Class (FSC) is S222 The associated Standard Industrial Classification (SIC) is 7359. The North American Industry Classification System is 562991 and the associated business size standard is 500 employees. This request for quotation consists of the following: Services Nonpersonal: Contractor shall provide all personnel, equipment, tools, materials, supervision, quality control, and other items necessary to perform cleaning and sanitation of 41, government furnished, portable chemical toilets. (See Statement of Work attached). STATEMENT OF WORK CLEANING OF PORTABLE TOILETS TRAVIS AFB CA 1.0 SCOPE OF WORK: This contract is for the cleaning of the portable toilets. The contractor shall provide all personnel, equipment, tools, materials, supervision, quality control, and other items and services necessary to complete this contract. Contractor shall comply with all federal, military and state and local laws and regulations. 2.0 HOURS OF OPERATION: All work will be performed on Wednesdays between the hours 0700 and 1600 hours. The contractor will report to the Utilities shop by 0800 before cleaning any latrines to receive locations and a checklist to follow. The Utilities shop will provide and escort for secure areas and to provide for quality control. Any deviations from this schedule will be scheduled with the 60 CES Utilities Shop. The Utilities Shop is located in building 176 on First Street. The phone numbers for the Utilities shop are 707-424-8869 and 707- 424-3025. The POC for any concerns should go through the Utilities Foreman Mr. Randy Lovejoy. 3.0 LOCATION: All work on this contract shall be performed on Travis Air Force Base (AFB), California. 4.0 DESCRIPTION OF WORK: The following outline of the principal features of work does not in any way limit the responsibility of the contractor to perform all work and furnish all materials required for a complete job, within the intent and scope of the contract documents. The following is a general description of the work requirements. 4.1 Contractor shall clean and pump waste from 41 portable toilets 4.2 Contractor shall replenish toilet tissue in units as needed. 4.3 Contractor shall report all problems with the units to the Utilities Shop Supervisor. 5.0 SITE CLEAN UP: The contractor shall remove all waste off base at the contractor expense. All waste shall be disposed of in an approved disposal site. 6.0 EQUIPMENT: The contractor shall provide all equipment and supplies required to complete the work, except for items specifically identified as being government furnished. 7.0 VEHICLES: The contractor shall provide all vehicles necessary to complete this contract. The contractor shall be able to transport all equipment, materials, supplies, and personnel to the job site and perform the required work. 8.0 WORKING CONDITIONS: Work shall be performed within facility in use by the government. All work shall be performed to minimize interference to existing use(s). The contractor shall comply with all federal, state, and local laws pertaining to safety and fire codes. 9.0 CONTRACTOR EMPLOYEES: The contractor?s foreman or representative shall have the capability of speaking and understanding the English language. This is a twelve month requirement from 1 Oct 2004 through 30 Sept 2005. Service shall be performed at Travis AFB, CA no later than 14 days after receipt of order. The following provisions and or clauses apply to this acquisition. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Commercial. FAR 52.212-2 Evaluation-Commercial Items, paragraph (a) to this provision is completed as follows: FAR 52.212-3, Offeror Representation and Certification-Commercial Items, a completed copy of this provision shall be submitted with the offer. Quotes without this provision will not be considered for an award. FAR 52.212-4, Contract Terms and Conditions-Commercial Items will be incorporated into any resultant Purchase Order by reference. FAR 52.212-5, Contract Terms and Condition Required to Implement Statues or Executive Orders-Commercial Items include clauses (b), (11), (12), (13), (14), (15), (16), (17), (22) (23) and (c).
