- Contractor to fill first time prescriptions for VA patients seen at the Dubuque Outpatient Clinic, located in Dubuque, Iowa, to provide medication counseling per OBRA 90 requirements to adhere to the VA formulatory, and to provide prescription pads to writing of these prescriptions for the period of 10/1/04 (date of award) - 9/30/05 with one year option included. Prices to be offered with respect to Average Whole Sale Price (AWP). Contractor must be ocated within (1-3) mile radius of the clinic. Request for quote solicitation can be downloaded at the internet site. The issue date will be 7/13/04. NAICS 44610. Size standard is $6 million. See note #1.
