Description
The Indian Health Service (IHS) is seeking to contract with a Medical Technologist to provide part-time, temporary services in the Laboratory at the Crow/Northern Cheyenne Indian Hospital, Crow Agency, Montana. The anticipated work schedule is 16-40 hours per week for night call, Holidays, and weekend call. The contractor must have ASCP with certification and a minimum of three years experience. The IHS intends to award a firm fixed-price purchase order as a result of this notice. The purchase order will be awarded pursuant to Federal Acquisition Regulation (FAR) 37.1 Personal Services Contract (Public Law (P.L.) 103-332, Department of Interior and Related Agencies Appropriation Act, Title II, September 30, 1994, 108 Stat. 2530, as implemented by 25 U.S.C. 1638c) and FAR Part 13, Simplified Acquisition Procedures. The contractor shall be subject to a character investigation as required by P.L. 101-630, Indian Child Protection and Family Violence Prevention Act and P.L. 101-647, the Crime Control Act of 1990. The qualification criteria against which the prospective contractors will be evaluated includes: (1) Resume; (2) Transcripts; (3) License/Certification; and (4) References. The award decision will be based on price and the contrators qualifications. The purchase order will be awarded for a 4-month period. Prospective contractors may request a copy of the statement of work by contacting Ms. Ruby Williamson, Contracting Officer, at (406) 638-3496, by faxing a request to (406) 638-3432, or by e-mailing Ruby.Williamson@mail.ihs.gov. Responses must include proposed hourly rate, resume, transcripts, copy of license/certification, and two references, and must be submitted by close of business on July 26, 2004. In addition, the contractor shall submit a completed copy of FAR provision 52.219-1, Small Business Program Representations (MAY 2004). The provision can be accessed electronically at http://www.arnet.gov. This acquisition is restricted to 100% Small Business concerns.
