Description
THIS IS NOT A SOLICITATION. VA Palo Alto is interested in determining the potential for Small Business participation in the awarding process for a contract which is expected to be awarded by Oct. 1, 2004. Details are as follows: Scope: The Contractor shall furnish all labor, transportation, parts and expertise necessary to provide preventive maintenance and repair of beds at VA Palo Alto Health Care System (VAPAHCS) and its divisions, Livermore and Menlo Park. Qualifications: a. To be considered eligible for consideration, bidders shall have been engaged in maintaining/servicing the equipment listed in Section B for a minimum of three (3) years. b. Bidders shall have a field service representative located in the San Francisco Bay area who has received maintenance training specific to the models listed in Section B. Bed manufacturers include Hill-Rom, Amedco, Nelson Ind., and Huntco. c. Bidders shall provide, upon request, evidence of appropriate training of any field service representatives providing services under the terms of this contract. d. Subcontracting of any ensuing award of this solicitation will not be allowed without written permission of the Contracting Officer. General Services to be Provided: a. Contractor shall provide the necessary manpower and supervision to properly execute the maintenance and repair program. b. Beds will be repaired or serviced within the designated areas. Parts and hardware will be replaced with manufacturer approved parts only. Operational checks will be performed and cosmetic repairs will be made. The beds will be labeled with a signed and dated preventive maintenance electrical safety inspection sticker. These labels MUST be of the type used by Biomedical Engineering and will be provided to the contractor. c. The Contractor shall maintain a parts inventory to service all beds with minimal delays. This inventory shall be replenished as needed to support the capabilities of the service contract. Only factory service parts shall be used. Defective parts replaced by the Contractor become the property of the Contractor. d. A monthly Bed Inspection Report shall be prepared and an electronic copy emailed to Contracting Officer's Technical Representative (COTR). This report will show the beds by: 1. Serial number, 2. Which beds have been serviced and 3. All parts replaced on each bed. The Contractor shall maintain a permanent record for all beds. An electronic copy of this report, listing beds by their locations and serial numbers shall be emailed to the COTR every quarter. e. The Contractor's service technicians shall provide on an as needed basis in-service training for new employees, or others, as deemed necessary by VAPAHCS. Training shall be coordinated with the VAPAHCS training/education department through the COTR. Repairs: Contractor shall respond by telephone within one hour from the time a VA telephone service call is placed. A technical maintenance representative shall arrive on-site for repair within one business day after a VA service call. Preventive Maintenance: Preventive maintenance shall be performed by the Contractor semi-annually for critical care beds and annually for other beds and shall, at a minimum, consist of the following: a. Contour Limit - When head is activated from a flat position, the knee will rise to form a "contour" position for the patient. Contour will be adjusted to manufacturer's specification as described in the service manual. b. Trendelenburg Limit - Activate Trendelenburg and reverse Trendelenburg functions. If cylinders require adjustment, refer to service manual. Inspect the degree indicators for accuracy. Lubricate the cylinder castings to ensure smooth operation of trend. c. Head Cylinders - Check for full travel of head section, inspect the cylinders for residual fluid. Check fittings for leaks; inspect angle of head elevation indicator for accuracy. d. Knee Cylinder - Check for full travel of knee section. Inspect for residual fluid. Inspect fittings for leaks. e. Hilo Cylinder - Check for full travel of Hilo Cylinder. Inspect the cylinder for residual fluid. Inspect the Hilo clevis pin for wear. Replace if necessary. f. Oil Reservoir - Remove cover. Inspect oil level in reservoir. If more oil is needed, refer to service manual. g. CPR Releases - With head and knees sections raised, activate CPR release. Both head and knee should lower simultaneously. h. Lockout switches - Each lockout shall be individually tested to ensure proper function. Master lockout switch shall also be tested. i. Central Brakes & Steers - Brakes shall be checked to ensure that the bed will not move when the brake is activated. Inspect brake pads for wear. Tighten pad if required. Steer shall also be checked. j. Caster Tires - Tires shall be checked for cuts, wear, tread life, etc. k. Side-Guard Controls - Switches in the side-guards shall be checked to determine condition. Intermittent operation shall also be checked at that time. l. Side-Guard Frame - The side-guard frame shall be inspected to insure proper latching of the side-guard in the various positions. m. Pivot Points - All pivot points on the bed shall be lubricated. n. Power Cord and Plug - The cord and plug shall be checked to ensure there are no cuts, nicks, or breaks. o. Motor capacitor shall be checked to ensure it is not weak or defective. p. Scale ? If bed is so equipped, check against known weight to ensure accuracy. q. Head and Foot Panels - Check aesthetics. r. Overall Appearance - Check general aesthetics and touch-up paint where necessary. Inspect labels and replace if necessary. s. Electrical Safety Test - Electrical safety tests shall be performed to ensure compliance with ground resistance and leakage current limits in the most recent version of NFPA 99, "Health Care Facilities". Strain relief and plug integrity shall also be evaluated. All deficiencies shall be corrected. t. Labeling - Upon completion of Preventive Maintenance and Electrical Safety Test, Contractor's service technician shall affix an inspection label provided to the Contractor by the COTR at the foot of the bed. u. Amedco beds, Model 2103 will need one major preventive maintenance inspection and three minor preventive maintenance inspections annually. Minor inspections will focus on the cleaning and adjustment of brake and steering casters. Documentation: A documentation package acceptable for JCAHO purposes shall be maintained by the Contractor and made available to VAPAHCS. Required features include, at a minimum: a. Inventory. The Contractor shall survey all designated areas within the first two months after contract award and annually to generate a computerized inventory of all beds to be serviced. The Contractor will email the electronic inventory to the COTR after completion. b. Service Histories. The database shall have the capability of providing a service history for each bed beginning at the contract award date. c. Preventive Maintenance. The Contractor shall perform PM according to manufacturer's specifications; to include at minimum the tests listed in Paragraph C.3.c. above. The Contractor shall have written procedures to be followed and documented evidence demonstrating that each bed has been inspected according to those procedures. New Equipment: Equipment may be added or deleted from the service contract as needed upon notification by the Contracting Officer (to be documented in writing via contract modification). Pricing of added equipment shall be the same unit price as established for the same model bed in Contractor's original bid or by agreement of Contractor and Contracting Officer if additional equipment is a model that is not readily covered by contract terms. Deleted equipment shall be credited in the full amount if deleted before any maintenance if repair has been performed on it. Documentation. At the conclusion of each repair and scheduled maintenance visit, the Contractor shall provide a written service report indicating the date of service, the model, serial number and location of equipment serviced, the name of the service representative, and the services performed and parts replaced.
