Description
Under the authority of Public Law 104-262 and 38 U.S.C. 8153, the VA Medical Center located at 7305 N. Military Trail West Palm Beach, Florida 33410-6400 seeks full and open competition for the award of a single contract for the services of a team comprised of an Interventional Radiologist (IR), Registered Nurse (RN) and Radiologic Technologist (RT), for after hours coverage, (4pm ? 8am) 7 days a week, including federal holidays. In addition, the Interventional Radiologist shall be required to provide services two (2) days per week during normal business hours (8am ? 4:30pm). Period of performance shall be a base year period with one (1) option year for renewal, which can be exercised at the discretion of the Government. The total duration of the contract shall not exceed twenty-four (24) months. All work shall be performed at the address referenced above. Response time for after hours shall be thirty (30) minutes. Procedures include but are not limited to: angiography, biliary drainage, percutaneous renal interventions, percutaneous abscess/fluid drainage, biopsies except for central nervous system, aortic stent grafts, carotid stents, central and peripheral venous and arterial stents, embolization procedures other than intracranial, caval filter placement, thrombolysis (mechanical and/or pharmacological), percutaneous gastrostomies, and gastrojejunostomies. The Government intends to award a firm fixed price contract in accordance with all terms, conditions, provisions, specifications and schedules of the solicitation, pending the availability of funds for FY ?05. Each offer should contain the offeror's best terms from a cost/price and technical standpoint, as contact award may be based on initial offers received without discussions. This solicitation is issued as a commercial item in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 15. Request for Proposal (RFP) 548-04-224 is advertised as full and open competition with NAICS: 621111 and a size standard of $8.5 million. The solicitation will issue on or about July 16, 2004. The closing date for submission of proposals will be on or about July 23, 2004 at 2:00PM local time. No telephone or fax requests for solicitation will be accepted.
