Description
THIS SOLICITATION IS BEING CONSIDERED FOR A TOTAL HUBZONE SMALL BUSINESS SET-ASIDE. Prime Contractors who are HUBZone small businesses and who intend to submit an offer for this solicitation, MUST indicate their intention in writing to the Indian Health Service (IHS), 1220 SW Third Avenue, Room 476, Portland, OR, 97204 or by facsimile to 503/326-7425, for receipt by close of business July, 23, 2004. Your submission must include the following: (1) a positive statement of your intention to submit an offer for this solicitation as a Prime Contractor; (2) your date of HUBZone small business certification; (3) a statement of your bonding capability, including both single and aggregate totals; and (4) a listing of your experience in work of similar type and scope, including contract numbers, project titles, dollar amounts, and the names and phone numbers of points of contact for the contracting entity. This agency will NOT contact respondents for missing data, so it is imperative that you include sufficient detail on all of the above points. Failure to submit complete information will eliminate a Contractor from consideration as an interested bidder for this solicitation. A decision on whether this project will be pursued as a HUBZone small business set-aside or on an unrestricted basis will be posted as a modification in FedBizOpps. THIS WORK CONSISTS OF improving the community sewer collection and treatment system on the Hoh Indian Reservation. THE PROJECT INCLUDES the construction and installation of approximately 1,291/LF of 4-inch PVC sewer main with cleanouts, location tape, and marker posts; four (4)/EA ? 4-inch cleanouts on existing sewer main; replace the septic tank inlet pipe on two (2) homes; rehabilitate ten (10) existing septic tanks; replace two (2) dosing siphons; renovate two (2) sand filters; remove existing trees and brush around and within filter and rapid infiltration basin (RIB) area; repair chain link fencing around filter and RIB area; hydro-seed all disturbed areas and necessary appurtenances for a complete and operational system. THE PROJECT IS LOCATED on the Hoh Indian Reservation, Jefferson County, Washington. The anticipated contract will be a firm-fixed price contract. The performance period is approximately 100 calendar days. The estimated PRICE RANGE is between $100,000 and $250,000. Performance and Payment bonds are required. Liquidated damages apply. The BUY AMERICAN ACT applies. Indian preference in hiring, suppliers and subcontractors is required, including meeting Hoh Tribal Ordinances.
